[摘要] 目的 探讨自体血清对白内障超声乳化术后干眼症的治疗作用。 方法 选取2014年3月～2015年2月在沈阳市第五人民医院眼科就诊行白内障超声乳化联合人工晶体植入术的患者92例，按照随机数字表法分为对照组和自体血清组，每组各46例。术后对照组常规滴用碘必殊眼药水，自体血清组滴用自体血清滴眼液进行辅助治疗。两组患者术后进行术眼症状、角膜荧光素染色、角膜知觉、泪膜破裂时间、基础泪液分泌量检测。 结果 治疗后，两组干眼症状平均评分和角膜荧光素检测平均评分与术后1 d未进行干眼治疗时比较均明显下降，术后1个月后，自体血清组干眼症状评分和角膜荧光素检测评分明显低于对照组，差异有统计学意义（P < 0.05）；治疗1周、1个月，自体血清组患者角膜知觉检测结果均优于对照组，差异有统计学意义（P < 0.05）；术后1周、1个月，自体血清组基础泪液分泌量、泪膜破裂时间均优于对照组，差异有统计学意义（P < 0.05）。 结论 自体血清对白内障术后患者干眼症状具有治疗作用。

Clinical effect of autologous serum for treatment of dry eye after phacoemulsification

[Abstract] Objective To investigate the effects of autologous serum on dry eye after phacoemulsification. Methods From March 2014 to February 2015， in the Fifth People' Hospital of Shenyang City， 92 patients with phacoemulsification combined with intraocular lens implantation were selected， according to random number table， they were divided into control group and autologous serum group， with 46 in each group. After the surgery， the control group was given conventional TobraDex eye drops， autologous serum group was given autologous serum. Dry eye symptoms， corneal fluorescence stain， corneal sensitivity， tear breakup time and SchirmerⅠtest of Two groups patients were detected. Results After treatment， dry eye symptoms mean scores and corneal fluorescence stain average scores in two groups were significantly lower than the first day after surgery without dry eye treatment （P < 0.05）， after surgery 1 month， the dry eye symptoms mean scores and corneal fluorescence stain average scores in autologous serum group were lower than that in the control group， the differences were statistically significant （P < 0.05）. Afetr surgery 1 week， 1 month， the result of corneal perception test in autologous serum group was better than that in the control group， the difference was statistically significant （P < 0.05）. Afetr treatment 1 week， 1 month， sehimer Ⅰ and BUT in autologous serum group were lower than that in the control group， the differences were statistically significant （P < 0.05）. Conclusion Autologous serum has therapeutic effect on dry eye symptoms in patients with phacoemulsification.

白內障为眼科常见病，是临床上常见的致盲性眼病之一[1]，目前主要治疗方式是手术。白内障超声乳化手术具有手术时间短、组织损伤小、切口愈合快等优点，得到普遍认可[2]。但由于术后诸多因素致使干眼症等疾病出现，影响患者恢复[3]。干眼症主要表现为眼干涩、异物感、视力模糊等，严重者将影响术后视力的恢复[4]。为减轻白内障术后干眼症状，选择合适的人工泪液至关重要。自体血清（autologous serum）是一种生理性泪液补充剂，是较理想的人工泪液，其对眼表具有润滑和营养的双重作用，具有天然、不易致敏等优点。临床一般只用于角膜并发症较明显的情况[5]。……
