加服米非司酮配伍米索终止早期妊娠临床效果分析

目的探讨药流在传统剂量上加服米非司酮的临床效果。方法收集该院2015年1—12月438例早孕要求药物流产的妇女病例，随机分为3组：对照组A为140例，服用米非司酮6片，配伍米索3片；研究组B共151例，服用米非司酮8片，配伍米索3片；研究组C共147例，服用米非司酮10片，配伍米索3片。服用后3 d查看绒毛，观察阴道流血，复查B超和血HCG下降。结果A组急性阴道大出血需清宫8人，占5.7%，高于B和C组；A组（38.6%）较B（7.3%）和C组（5.4%）的阴道流血时间长，清宫率高。结论加服米非司酮后，联合米索终止早孕完全流产率高，阴道出血少，清宫率低，临床取得满意效果。

［Abstract］Objective To discuss the clinical effect of medical abortion in the traditional dose of mifepristone.Methods Collected 438 cases of early pregnancy abortion women in our hospital from January to December in 2005，were randomly divided to 3 groups，the control group A （n=140）taking mifepristone 6 pieces，combined with misoprostol 3 tablets；research group B（n=151）taking mifepristone 8 pieces，combined with misoprostol 3 tablets；research group C （n=147）taking mifepristone 10 pieces，combined with misoprostol on 3.View the villi after 3 days，observation of vaginal bleeding，review of B ultrasound and blood HCG decreased.Results The acute vaginal bleeding in group A （38.6%），there are 8 people requiring emergency uterine curettage，accounted for 5.7%，than group B（7.3%）and C （5.4%）.control group A than study group B and C vaginal bleeding time long，dilatation and curettage rate high.Conclusion Mifepristone combined with misoprostol to terminate early pregnancy abortion rate is high，less bleeding，lower rate of evacuation，clinical result is satisfactory.

米非司酮与米索前列醇已成为最常用的药流方案［1-4］。传统米非司酮药物流产不良反应较多，出现阴道大出血急诊清宫以及不全流产清宫时有发生，完全流产成功率低，阴道出血时间长，病人对此不满，主观上医生也不愿给早孕妇女做药流，而是更倾向于无痛人流，出血少，时间短，医患纠缠少。随着多次妊娠的增多，疤痕子宫再次妊娠，还有胎盘部位滋养细胞肿瘤时有发生，人流手术病人也随时会发生术中出血不止危及生命需要急救的情况，增加了不安全事件的发生，如何更加健康有效地减少流产中不安全因素是妇科大夫时常思考、探索的课题。……
