64排螺旋CT联合超敏C-反应蛋白术前评估胃癌患者TN分期的临床价值

目的探索研究应用64排螺旋CT联合超敏C-反应蛋白于术前评估胃癌患者TN分期的作用和效果。方法将该院2015年5月—2016年5月期间收治的84例胃癌患者作为研究对象并实施随机分组，对照组40例仅以64排螺旋CT检查，研究组44例则以64排螺旋CT联合超敏C-反应蛋白检查。两组分别确定患者的TN分期，将分期结果与术后病理检查结果进行对比，同时进行组间比较。结果将两组患者事前分期检测与术后病理学分期进行对比分析，研究组患者T分期准确率为70.45%，与对照组的67.50%相对接近（P＞0.05），差异无统计学意义；研究组N分期准确率84.09%，显著性超过对照组的65.00%（P＜0.05），差异有统计学意义。结论应用64排螺旋CT联合超敏C-反应蛋白积极开展术前评估。可以有效提升胃癌患者TN分期的准确率，值得推广应用。

［Abstract］Objective To probe on the clinical value of combined use of 64-row spiral CT with super-allergic C-reactive protein test for preoperative TN staging in patients with gastric cancer.Methods Of the 84 patients with gastric cancer who were hospitalized in our hospital between May 2015 to May 2016，40 cases were randomly divided into the observation group and 44 cases into the control group.For the control group，only 64-row spiral CT was used for examination；for the research group，combined use of 64-row spiral CT examination with super-allergic C-reactive protein test was applied.The TN staging for the two groups were respectively determined and a comparison was made between the staging result and the result of postoperative pathological examination.A comparison was also made between the two groups.Results A comparative analysis was made between the pre-staging examination and postoperative pathological staging of the patients.The accuracy rate of patients’T staging of the observation group was 70.45%，the accuracy rate of the control group was 67.50%，and the levels of the two are close（P＞0.05）；the accuracy rate of N staging of the observation group（84.09%）was obviously higher than that of the control group（65.00%），and the difference was statistically significant（P＜0.05）.Conclusion The preoperative assessment of the combined use of 64-row spiral CT with super-allergic C-reactive protein test can effectively improve the accuracy of TN staging of patients with gastric cancer. It is worthy of wide application.

一直以来，胃癌是人类消化系统中比较多见的恶性疾病，在消化道肿瘤疾病中胃癌的死亡率所占比例最高［1］。其死亡率居世界癌症死亡率的第2位［2］。在目前条件下，针对该病实施治疗，最好方法为手术，同时辅助药物治疗。准确进行术前分期，对于选择对应的手术方式具有积极意义。该院积极探索研究应用64排螺旋CT联合超敏C-反应蛋白于术前评估胃癌患者TN分期的作用和效果，于2015年5月—2016年5月针对84例患者进行了相关实践操作，取得了较为满意的经验，现报道如下。……
