经脐单孔腹腔镜胆囊切除术治疗急性胆囊炎的效果评价

目的分析经脐单孔腹腔镜胆囊切除术治疗急性胆囊炎的效果。方法随机选取所在医院2013年3月—2015年7月期间收治的70例急性胆囊炎患者作为观察对象，所有患者均接受腹腔镜手术，根据手术入路方法进行随机分为2组，观察组35例经脐单孔开展相关手术治疗，对照组35例采用三孔法实施手术治疗，比较2组患者手术治疗情况。结果 观察组手术时间（45.63±2.74）min、术中出血量（67.89±10.47）mL、下床活动时间（2.33± 0.55）d、住院时间（6.82±2.52）d，对照组手术时间（46.05±2.66）min、术中出血量（68.52±10.36）mL、下床活动时间（2.29±0.61）d、住院时间（6.91±2.34）d，2组手术时间、术中出血量、活动时间、住院时间差异无统计学意义（P＞0.05）；观察组术后12 h内（7.82±2.34）分、术后3 d（4.20±1.46）分、术后7 d（2.04±0.52）分疼痛评分明显低于对照组术后12 h内（8.81±2.35）、术后3 d（7.78±2.05）、术后7 d（6.34±1.56）疼痛评分，2组差异有统计学意义（P＜0.05）。结论2种入路方法临床疗效相当，但是与常规三孔法相比，经脐单孔腹腔镜胆囊切除术疼痛感较轻，对患者预后具有积极影响，值得临床推广。

［Abstract］Objective Analysis of transumbilical single port laparoscopic gallbladder resection in treatment of acute cholecystitis.Methods Randomly selected hospital from March 2013 to July 2015 70 cases of patients with acute cholecystitis patients as the observation object.All patients underwent laparoscopic surgery，according to the surgical approach for were randomly divided into two groups，the observation group of 35 cases of transumbilical single port to carry out related to surgical treatment and control group 35 cases were treated by three holes method implementation of surgical treatment and surgical treatment were compared between the two groups.Results The operation time of the observation group（45.63±2.74）min，intraoperative hemorrhage volume（67.89±10.47）mL，get out of bed activity time（2.33± 0.55）d，hospitalization time（6.82±2.52）d.Control group operation time（46.05±2.66）min，intraoperative hemorrhage volume（68.52±10.36）d，get out of bed activity time（2.29±0.61）d，length of hospital stay（6.91±2.34）d，two groups of operation time，intraoperative bleeding volume，activity time，hospitalization time，there were no significant difference（P＞0.05）；observation within 12 hours after operation in group 12 h（7.82±2.34），postoperative 3 d（4.20±1.46），Postoperative 7 d（2.04±0.52）pain score was significantly lower than that in the control group after 12 h（8.81±2.35），Postoperative 3 d （2.05±7.78），postoperative 7 d（6.34±1.56）pain score，the difference between the 2 groups was significant（P＜0.05）. Conclusion Two kinds of approach to the clinical curative effect quite，but compared with the conventional method of three holes，transumbilical single port laparoscopic gallbladder resection surgery pain feel less，on the prognosis of the patients with positive effect，it is worth clinical promotion.
