腹腔镜阑尾切除术阑尾残端两种处理方法的对比研究

目的对比研究hem-o-lok夹闭与结扎后和荷包缝合包埋在腹腔镜阑尾切除术当中的应用效果。方法选择2013年6月—2015年8月之间因急性阑尾炎就诊于该院并行腹腔镜阑尾切除术的患者130例，应用随机数字表法将所有患者随机分为两组，实验组66例，采用hem-o-lok夹闭法处理阑尾残端，对照组64例，在阑尾根部行丝线结扎后，行荷包缝合包埋残端，对比两组患者的手术时间、住院时间、手术费用以及并发症情况。结果实验组患者手术时间为（30.15±12.37）min，短于对照组的（57.88±13.26）min，实验组的手术费用为（8267.53±226.41）元，高于对照组的（5321.22±176.62）元，差异均有统计学意义（P＜0.05）。两组患者的住院时间以及并发症发生率差异无统计学意义（P＞0.05）。结论两种处理方法，均安全有效，应根据术者技术水平以及患者经济条件进行选择。

［Abstract］Objective to contrast the hem-o-lok clip and ligation and pouch suture after embedding the application effect of laparoscopic appendectomy.Methods June 2013—August 2015 between parallel laparoscopic appendectomy for acute appendicitis clinic in our hospital 130 cases of patients，the application of random number table method，all of the patients were randomly divided into two groups，the treatment group 66 examples，the hem-o-lok clip method and treatment of appendix stump，in the control group of 64 cases in the appendix root silk after ligation，pouch suture of the embedding stumps，compared two groups of patients with operation time，length of hospital stay，operation cost and complications.Results Experimental operation time patients was（30.15±12.37）min，shorter than the control group（57.88±13.26）min，the experimental operation cost is（8267.53±226.41）yuan，higher than the control group （5321.22±176.62）yuan，the difference had statistical significance（P＜0.05）.The length of hospital stay and complications in patients with similar between the two groups has no statistical significance（P＞0.05）.Conclusion Two methods of processing，all is safe and effective，should be based on performer technology level and economic conditions to choose.

在腹腔镜阑尾切除术中，阑尾残端的处理至关重要，目前，对于阑尾残端的处理方式临床上尚有争议。有学者认为，在阑尾系膜处理完毕后对阑尾残端进行简单的夹闭即可［1-2］，但有学者持反对的观点，认为应实施荷包缝合包埋［3-5］。该实验选取了2013年6月—2015年8月之间因急性阑尾炎就诊于该院并行腹腔镜阑尾切除术的患者130例，旨在探究腹腔镜阑尾切除术中阑尾残端的处理方式，现报道如下。
