摘 要 目的：通过医养结合服务模式的实施，评价医养结合服务模式对促进老年人身心健康的效果。方法：2013年6月光明社区卫生服务中心与辖区内3家社会养老机构结对，签订服务协议，由家庭医生每周1次到养老机构提供查房、医疗咨询、老年预防保健等医疗服务。每月提供1次健康讲座和慢性病随访。采用SF-36生活质量评定表对实施前后老年人生活质量进行评分和比较。结果：通过2年来医养结合服务模式的实践，养老机构中老年人的生活质量、慢性病的随访率及控制率得到明显提高。结论：医养结合服务模式是一种新型的养老服务模式，能有效促进老年人的身心健康和生活质量，应积极推广应用。

Preliminary exploration and evaluation of the mode of health service combined with caring for the elderly in Guangming Community

ABSTRACT Objective： Through the preliminary exploration of the mode of health service combined with caring for the elderly， to evaluate and promote the elderly physical and mental health in the nursing homes to promote the effect of their life quality. Methods： In June 2013， Guangming Community Health Service Center twinned with 3 social nursing homes and signed the service agreements that the family doctors provided the rounds of the wards， medical advice， elderly prevention and health care and other medical services once a week， and gave the health lectures， and follow-up of chronic disease once a month. The F-36 life quality evaluation scale was used to evaluate and compare the living quality conditions of the elderly before and after implementation of the mode of the health service combined with caring for the aged. Results： Through two years of preliminary practice of the mode of health service combined with caring for the aged， the condition of the elderly living quality， follow-up and control rates of chronic diseases were significantly improved in the nursing homes. Conclusion： Practice of the model of health service combined with caring for the aged is a new mode that can let more elderly people have health service and caring for， and has the great significance to promote the physical and mental health and improve the life quality of the elderly.

我国人口老龄化形势严峻，“第六次全国人口普查”数据显示，2010年大陆60岁及以上人口占13.26%，其中65岁及以上人口为1.19亿人，占8.87%，中国已经进入“老年型社会”，2013年，老年人口数量突破2亿，老龄化水平将达到14.8%[ 1 ]。让老年人老有所医、老有所养、提高生活质量和幸福感必须探索一种医养结合的新型养老服务模式，也是国际社会总结发达国家几十年来解决老年医养结合经验教训所得出的重要结论[ 2 ]。本文就近2年来本社区卫生服务中心联合辖区内3家社会养老机构推行医养结合服务模式的初步实践进行效果评价。
