针刺背俞穴治疗更年期抑郁症临床观察

摘要：目的观察针刺背俞穴为主治疗更年期抑郁症的临床疗效和不良反应。方法将60例患者随机分为治疗组30例和对照组30例，治疗组给予背俞穴为主的针刺治疗，主穴肺俞、心俞、膈俞、肝俞、脾俞、胃俞、肾俞,针刺补法；对照组口服尼尔雌醇(2 mg，每半月1次)加盐酸氟西汀胶囊(20～40 mg，1次/d)。10 d为1个疗程,2组分别观察3个疗程。采用HAMD量表为评定工具,分别于患者治疗前、治疗1、2、3个疗程后各评定1次,评价治疗效果和不良反应观察。结果2组HAMD总分均逐渐下降，治疗组在治疗前、治疗1、2、3个疗程后的HAMD总分分别为:(26.97±5.13)、(23.59±6.15)、(16.13±6.93)、(11.73±5.58),对照组的HAMD总分分别为：(27.15±3.69)、(23.18±4.56)、(19.08±1.30)、(15.32±3.86)，在2、3个疗程后,治疗组的总分明显低于对照组(P<0.05)，差异有统计学意义；2组间的总有效率有显著性差异(P<0.05)，差异有统计学意义，治疗组总有效率为93.33%，对照组为86.67%，治疗组优于对照组。结论针刺背俞穴为主从补益与调理脏腑入手,改善更年期抑郁症患者的症状。

Clinical observation of acupuncture of beishu point in treatment of climacteric depression

Abstract:ObjectiveTo observe the clinical efficacy and adverse reactions of acupuncturing beishu point in the treatment of climacteric depression.MethodsThe 60 patients were randomly divided into the treatment group (n=30) and control group (n=30),the treatment group was given acupuncture treatment of beishu point,The chief points were feiyu,xinyu,geyu,ganyu,piyu,weiyu,shenyu,Acupuncture method was used as reinforcing method;the control group was given oral Neal estradiol(2 mg,every half a month) plus fluoxetine hydrochloride capsules(20-40 mg，once a day).10 days were a course of treatment and two groups were observed for 3 courses.HAMD scale was used as assessment tool to evaluate the score of two group in patients before treatment and 1,2,3 course after treatment respectively and to evaluate the curative effect and adverse reactions.ResultsThe total scores of HAMD in two groups were both gradually decreased,the HAMD score of treatment group before treatment,1,2,3 months after treatment respectively were:(26.97±5.13),(23.59±6.15),(16.13±6.93),(11.73±5.58),and the HAMD scores of the control group were:(27.15±3.69),(23.18±4.56),(19.08±1.30),(15.32±3.86) after the 2 and 3 courses of treatment,the total score of treatment group was lower than that of control group (P<0.05);the two groups of the total efficiency had significant difference (P<0.05),the total effective rate of treatment group was 93.33%,control group was 86.67%,the treatment group was better than the control group.ConclusionAcupuncturing at beishu points starts from nourishing and conditioning organs,and fundamentally improves the symptoms of climacteric depression patients.

更年期抑郁症以情绪低落、心悸失眠、多疑善虑、焦虑不安、悲观消极、性功能减退等为主要症状[1]，常有明显的神经质、恐怖、紧张、惊恐、烦躁不安、心悸等焦虑症状[2]，严重者有不同程度的轻生念头[3]。临床上常用抗抑郁药治疗，但毒副作用大，易成瘾。笔者应用针刺五脏俞穴治疗此病，报告如下。

1临床资料

1.1一般资料60例患者来源于2011年6月—2014年6月到本院就诊病患,年龄46～58岁,病程1个月～3年。……
