参芪降糖颗粒对早期糖尿病肾病的治疗作用

摘要：目的探讨参芪降糖颗粒治疗早期糖尿病肾病(DN)的临床效果及安全性。方法选取早期糖尿病肾病患者60例，随机分为治疗组与对照组,各30例。2组均进行饮食控制，进低盐、低脂、优质低蛋白饮食，给予胰岛素、二甲双胍口服控制血糖，并依据患者血糖控制情况酌情加用瑞格列奈或者阿卡波糖口服治疗，控制餐后2 h血糖≤10 mmol/L，空腹血糖≤7 mmol/U。治疗组患者在上述常规治疗的基础上加服参芪降糖颗粒治疗，对照组仅行常规治疗，对比2组患者治疗前及治疗12周后血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1)、血脂(TC)、24 h尿微量白蛋白定量等指标改善情况。结果治疗组治疗后24 h尿微量白蛋白定量、FPG，2 hPG及HbA1均显著低于对照组(P<0.05)，2组治疗后血脂相关指标均较治疗前显著下降(P<0.05)，2组治疗过程中均未发生明显不良反应。结论参芪降糖颗粒联合常规治疗，可有效降低早期DN患者血糖指标及24 h尿微量白蛋白定量，改善患者肾功能作用明显。

Therapeutic effect of Shenqi Declining Sugar Granules on early diabetic nephropathy

Abstract：ObjectiveTo explore the clinical efficacy of Shenqi Sugar Declining Granules in treating early diabetic nephropathy (DN) and its safety.MethodsA total of 60 early DN patients were enrolled in our study and randomized into the treatment group and the control group with 30 cases in each group.The patients in the two groups were all given diet control,i.e.low salt,low fat,low protein of high quality diets.They were orally administered with insulin and metformin to control the blood sugar,and the repaglinide or acarbose was appropriately added according to the condition of blood sugar control.The blood sugar 2h after a meal was controlled to be less than 10 mmol/L,and the fasting blood sugar was less than 7 mmol/U.Based on the conventional treatments,Shenqi Sugar Declining Granules were added to the patients in the treatment group,while the patients in the control group were only given conventional treatments.The improvements of FPG,2 hPG,HbA1,TC,and 24 h urine microalbumin before treatment and 12 weeks after treatment in the two groups were compared.ResultsThe 24 h urine microalbumin,FPG,2 hPG,and HbA1 after treatment in the treatment group were significantly lower than those in the control group (P<0.05).The serum lipid related indicators after treatment in the two groups were significantly reduced when compared with those before treatment (P<0.05).No obvious adverse reactions occurred during the therapeutic process in the two groups.ConclusionShenqi Sugar Declining Granules combined with conventional treatments can effectively reduce blood sugar indicators and 24 h urine microalbumin in the early DN patients,and obviously improve the patients’ renal functions.

糖尿病(DM)为临床常见病，随着现代生活、饮食习惯及环境的变化呈逐年高发趋势[1-3]。糖尿病肾病(DN)又称糖尿病性肾小球硬化症，是DM终末期常见微血管并发症之一，已经升至终末期肾脏病(ESRD)病因的首位，并且是DM患者主要的致死原因[4]。有研究[5]证明，在DN早期通过正确的治疗，可延缓或逆转其病理过程。因此，早期诊断及有效治疗，对改善早期DN患者的预后意义重大。……
