临床路径在儿童轮状病毒肠炎治疗中的应用

目的：探索临床路径在儿童轮状病毒肠炎的实施效果。方法采用回顾性调查分析的方法，将实施临床路径后102例轮状病毒肠炎病例作为实验组，上一年同期收治的以轮状病毒肠炎作为诊断的单一疾病的128例病例作为对照组，分别统计其平均住院天数、平均住院费用、使用抗生素比例、患儿家属满意率、签字出院率等临床资料，并进行统计学分析。结果2组平均住院天数差异无统计学意义(P>0.05)；实验组平均住院费用、使用抗生素比例和签字出院率低于对照组，实验组患儿家属满意率高于对照组，差异均有统计学意义(P<0.05)。结论轮状病毒肠炎引入临床路径实施诊疗、护理可明显降低住院费用，提高医疗质量，提升医疗护理服务满意率。

Abstract：[Objective] To evaluate the effects of application of clinical pathway in puerile rotavirus enteritis. [Method] 102 patients in the experiment group were managed in clinical pathway, while 128 patients in the control group were managed in traditional methods. The average hospital residence day and charges, the use of antimicrobial drugs ratio, the patient satisfaction and the signature of discharge ratio were compared and statistically analyzed between two groups. [Result] After the clinical pathway application, the average hospital charges, ratio of the use of antimicrobial drugs, and the signature of discharge ratio decreased. Satisfactory rate of the patients enhanced. However, there was no significant difference in hospital residence day between the two groups. [Conclusion] Application of clinical pathway in diagnosis and treatment of rotavirus enteritis can significantly reduce the average hospital charge, improve the health care quality as well as the medical care service satisfaction.

临床路径( Clinical Pathways，CP)是指由医疗、护理及相关专业人员在疾病诊断明确以后，针对某种疾病或某种手术制定的具有科学性(或合理性)和时间顺序性的患者照顾计划[1]。2011年4月开始我院对轮状病毒肠炎患儿实施临床路径管理，取得显著效果，现报告如下。

1 资料与方法

1．1 一般资料

将我院2011年4月至2012年02月实施临床路径管理以来的102例轮状病毒肠炎患儿作为试验组，男61例，女41例，年龄6～34个月，平均(15.1±6.3)个月。选择上一年同期收治的以轮状病毒肠炎作为诊断的单一疾病病例共128例作为对照。男76例，女52例，年龄6～36个月，平均(14.8±6.6)个月。2组病例均符合轮状病毒肠炎诊断标准[2]，均有蛋花样或水样大便，轻中度脱水，入院前查大便轮状病毒抗原阳性，排除肠道外感染及循环衰竭者。2……
