【摘要】 目的：研究分析“温阳祛瘀”中药熏洗联合复方益母草胶囊对阳虚血瘀型子宫复旧不全产妇早期康复的临床效果。方法：前瞻性纳入2021年4月—2023年4月在广东省人民医院赣州医院妇产科住院的阳虚血瘀型子宫复旧不全产妇80例，将患者按照随机数字表法分为治疗组（n=40）和对照组（n=40）。对照组在常规治疗的基础上加用复方益母草胶囊，治疗组在对照组基础上加以“温阳祛瘀”中药熏洗治疗。比较两组临床症状消失时间、子宫恢复情况、疼痛情况、睡眠质量、心理状态、临床疗效。结果：治疗组恶露、腹痛消失时间均早于对照组，子宫底高度、子宫体积均小于对照组，子宫腔积液量少于对照组，视觉模拟评分法（VAS）、抑郁自评量表（SDS）、焦虑自评量表（SAS）、匹兹堡睡眠质量指数（PSQI）评分均低于对照组，临床疗效优于对照组，差异均有统计学意义（P<0.05）。结论：“温阳祛瘀”中药熏洗联合复方益母草胶囊能显著促进阳虚血瘀型子宫复旧不全产妇早期康复，有效提高产妇睡眠质量，改善不良情绪，整体治疗效果较佳。

[Abstract] Objective： To study and analyze the clinical effect of early rehabilitation of subinvolution parturients with syndrome of yang deficiency and blood stasis by fumigation and soaking with "warming Yang and dispelling stasis" Chinese materia medica combined with Compound Yimucao Capsules. Method： A total of 80 subinvolution parturients with syndrome of Yang deficiency and blood stasis who were hospitalized in Department of Obstetrics and Gynecology， Guangdong Provincial People's Hospital Ganzhou Hospital from April 2021 to April 2023 were prospectively included. Patients were divided into treatment group （n=40） and control group （n=40） according to random number table method. The control group was treated with Compound Yimucao Capsules on the basis of conventional treatment， and the treatment group was treated with fumigation and soaking with "warming Yang and dispelling stasis" Chinese materia medica. The disappearance time of clinical symptoms， uterine recovery situation， pain situation， sleep quality， mental state and clinical efficacy of the two groups were compared. Result： The disappearance time of lochia and abdominal pain in the treatment group were earlier than those in the control group， the fundal height of the uterus and the volume of the uterus were smaller than those in the control group， the amount of fluid in the uterine cavity was smaller than that in the control group， the scores of visual analogue scale （VAS）， self-rating depression scale （SDS）， self-rating anxiety scale （SAS） and Pittsburgh sleep quality index （PSQI） were lower than those in the control group， and the clinical efficacy was better than that in the control group， the differences were statistically significant （P<0.05）. Conclusion： Fumigation and soaking with "warming Yang and dispelling stasis" Chinese materia medica combined with Compound Yimucao Capsules can significantly promote the early rehabilitation of subinvolution parturients with syndrome of Yang deficiency and blood stasis， effectively improve the quality of sleep and improve bad emotion， and the overall treatment effect is good.

子宫复旧不全是指产妇在分娩后，由于子宫体肌纤维收缩作用出现障碍，导致子宫体肌纤维不能按时缩复，引起下腹坠痛、恶露时间过长且血量增多等症状[1-2]。此外，产后激素水平的变化使产妇敏感且情绪不稳定，容易出现焦虑和抑郁等心理问题[3]。复方益母草胶囊具有调经活血、化瘀生新的功效，可以治疗子宫复旧不全所致的恶露不净，但效果具有一定局限性。中医认为，产后妇女常表现为“多虚多瘀”体质，寒邪易乘虚而入，因寒凝血瘀而引起产后腹痛[4]。……
