摘 要 抗凝临床药师分别为住院、门诊和居家患者提供闭环药学管理，制定药学服务流程，并在每个环节设置相应的服务内容，从而实现全程化药学服务。在住院期间提供查房、用药监护、床旁检测和用药咨询教育服务；在门诊提供咨询、检测和处方调整服务；在居家时间提供随访、提醒服务。该模式可作为抗凝临床药师的工作参考，可提高患者抗栓的依从性和规范性，充分发挥抗凝临床药师的价值，减少抗凝患者栓塞和出血事件，有较好的经济效益和社会效益。

Discussion on the integrated anticoagulant pharmaceutical care mode*

ABSTRACT Anticoagulant clinical pharmacists provide closed-loop pharmaceutical management for inpatients， outpatients and home patients， formulate pharmaceutical care process， and set corresponding service content in each link so as to realize the integrated pharmaceutical care. We provide ward round and medication monitoring for hospitalized patients bedside testing and medication consultation and education services， and consultation， testing and prescription adjustment services for outpatient and follow-up and reminder services for home patients. This mode can be used as a reference for the work of anticoagulant clinical pharmacists， improve the compliance and standardization of patients with antithrombotic therapy， give full play to the value of anticoagulant clinical pharmacists， reduce the embolism and bleeding events of anticoagulant patients， and has better economic and social benefits.

隨着人们生活水平的提高和生活方式的改变，血栓相关性疾病呈上升趋势，伴随抗栓治疗的日益广泛，各种问题也接踵而来，而患者对抗栓治疗认识不足和依从性不高、患者药物基因组学存在差异等一系列问题，造成患者再次栓塞风险升高和出血不良事件的发生[1-2]。由于对患者系统管理的缺乏，这为抗凝临床药师提供了工作切入点[3]，抗凝临床药师可以通过临床药师查房、抗凝门诊和居家随访为患者提供住院、门诊和居家的闭环管理，实现全程化抗凝药学服务。

1 全程化抗凝药学服务的软件和硬件建设支撑

1.1 人才团队建设

由3名及以上的抗凝专业临床药师组成专职服务团队，人员需有临床药师岗位资格证，形成高级职称和中级职称结合的人员梯队，建立一个结构合理、能力较强、执行到位和充满为患者服务热情的团队。……
