摘 要 目的：探讨临床使用大环内酯类抗菌药物不良反应的规律及特点，为临床安全用药提供参考。方法：收集2016年1月—2019年12月本院临床各科室上报的362例大环内酯类药品不良反应，统计分析患者基本信息、药物用法用量、不良反应累积器官-系统及临床表现、不良反应发生时间、处理情况和预后等。结果：所有例病报告中，男性患者略多于女性；静脉滴注发生的药物不良反应较多，最常见的表现为皮疹、瘙痒、静脉炎，严重的不良反应为过敏性休克甚至危及生命。结论：这些研究结果可为临床医护人员提供药物不良反应信息，保障临床安全、合理用药。

Analysis of adverse reactions in clinical application of macrolides

ABSTRACT Objective： To explore the rules and characteristics of adverse reactions of macrolides in clinical use and provide references for safe drug use in clinic. Methods： The data from 362 cases of adverse drug reactions caused by macrolides were selected and their basic information， drug usage and dosage， adverse reactions involved in organ-system and clinical manifestations， time for the occurrence of adverse reactions， treatment status and prognosis were statistically analyzed. Results： In all case reports， there were slightly more male patients than females， intravenous infusion had more adverse drug reactions， the most common manifestations were rash， pruritus， phlebitis， and the serious adverse reactions were anaphylactic shock or even life-threatening. Conclusion： These results can provide information on adverse drug reactions for clinical medical staff so as to ensure clinically safe and rational medication.

大环内酯类药物是一类具有大环内酯环基本结构的抗生素，自1952年发现并应用到临床以来，主要作用于革兰阳性菌、阴性菌及非典型病原体，具有适用人群广、半衰期长、服用方便、口服吸收好、毒性低微等特点[1]，在抗感染领域发挥了重要作用。随着大环内酯类药物在临床的广泛应用，其有关的药物不良反应（ADR）报道日益增多。明确大环内酯类抗菌药物用药后不良反应的发生类型对于其临床预防有非常重要的作用[2]。

1 资料与方法

1.1 资料来源

根据焦作市第二人民医院（以下简称“本院”）药房药库系统资料及国家不良反应监测中心数据[3]，统计分析本院2016年1月—2019年12月临床科室上报的362例大环内酯类药物不良反应，使用Excel软件对这些病例报告进行汇总[4]，每一例病例报告包含患者性别、年龄、用药通用名称、给药方法、不良反应发生情况等信息，并将这些病例信息进行分类统计。……
