摘 要：文章依据英、美、日、加的电子健康档案标准化建设构想及具体实施思路，总结概括出电子健康档案标准化建设的三种实现路径，即“自上而下型”“自下而上型”“中间型”的标准化路径，并从成本效益、实现效果等角度分析三种路径的优缺点，最后结合我国电子健康档案建设标准化现状，从价值重塑、路径选择、组织架构、标准规范、人才支撑方面提出我国电子健康档案标准化建设对策。

Commentary on the Realization Path of Electronic Health Records Standardization in China and Foreign Countries

Abstract：Based on the construction concept and specific implementation ideas of the standardization of electronic health records in Britain， the United States， Japan and Canada， this paper summarizes the triple standardization path of electronic health records， namely "top-down"， "bottomup" and "intermediate" standardization path， and analyzes the advantages and disadvantages of the three paths from the perspective of cost-effectiveness and implementation effect This paper puts forward the Countermeasures for the standardization of electronic health records in China from the aspects of value remodeling， path selection， organizational structure， standard specification and talent support.

电子健康档案（Electronic Health Records， EHR）是以居民个人为中心形成的覆盖全生命周期、完整可靠的数字化健康信息记录。电子健康档案作为医疗健康信息化的基础性资源支撑，其完善程度及现实推进影响制约着医疗健康信息化的步伐及程度，更事关公民个人基本权益以及国家社会的发展稳定。2020年暴发的新型冠状病毒肺炎疫情严重威胁人民群众生命健康，也给社会的长期平稳发展带来重大影响。在后疫情时代，反思既往，电子健康档案其特有价值在一定程度上得到了彰顯和印证。为践行健康中国“2030”规划纲要，严格贯彻“促进健康为中心”的“大健康观”“大卫生观”思想理念，全方位、全生命周期维护人民群众健康[1]，建立规范合理的跨部门、跨系统、跨区域电子健康档案信息共享平台是卫生信息化建设的目标，也是电子健康档案价值实现的重要基础。

现阶段我国电子健康档案的区域性试点建设工程取得了初步性成果，如浙江省杭州市已于2017年建成了覆盖全市的卫生信息平台，实现居民电子健康档案实时源头采集、互通共享。……
