[摘要] 目的 通过综合性分析辽宁某医院门诊中药饮片处方情况，以规范中药饮片处方，提高处方质量，促进临床合理应用。 方法 随机抽取辽宁某医院2017—2019年门诊中药饮片处方7200张进行点评，并分析用药情况及存在的问题。 结果 7200张中药饮片处方中，患者年龄集中在55～65岁，就诊科室以胃肠病科、妇科居多，5～7剂处方剂数、16～20味单张处方味数及>60元且≤80元单剂处方金额偏多;存在不合理处方397张，处方不合理率为5.51%。 结论 门诊中药饮片处方存在不合理使用情况。通过处方点评可进一步指导临床合理用药水平，提高中药饮片处方质量，保证患者用药安全、有效、经济。

Review of 7200 outpatient prescriptions of Chinese herbal decoctions in a hospital in Liaoning

[Abstract] Objective To comprehensively analyze the prescription of Chinese herbal decoctions in the outpatient clinic of a hospital in Liaoning， so as to standardize the prescription of Chinese herbal decoctions， improve the quality of prescriptions， and promote rational clinical application. Methods A total of 7200 outpatient prescriptions of Chinese herbal decoctions in a hospital in Liaoning from 2017 to 2019 were randomly selected for review， and the medication and existing problems were analyzed. Results Among the 7200 prescriptions of Chinese herbal decoctions， the patients' ages were around 55-65 years old. The majority of medical departments were gastroenterology and gynecology. The number of 5-7 prescriptions， the number of 16-20 ingredient single prescription and the cost of single prescription more than 60 yuan and less than or equal to 80 yuan were relatively high; there were 397 unreasonable prescriptions， and the unreasonable prescription rate was 5.51%. Conclusion There is an unreasonable use of prescriptions of Chinese herbal decoctions in outpatient clinics. Prescription reviews can further guide the level of clinical rational medication， improve the quality of prescriptions of Chinese herbal decoctions， and ensure the safety， effectiveness， and economy of patients′ medication.

處方是医师在诊疗活动中为患者开具，由药师审核、调配，作为患者用药凭证的医疗文书，开具门诊处方作为患者在诊疗过程中的重要环节，对于保障患者的用药合理性和规范性具有重要意义[1]。2015年10月，国家中医药管理局印发了《关于进一步加强中药饮片处方质量管理强化合理使用》的通知，明确要求建立健全系统化、标准化和持续改进的中药饮片处方专项点评制度，定期和不定期对中药饮片处方书写的规范性、药物使用的适宜性、每剂味数和费用进行评价。本研究通过综合性分析辽宁某医院2017—2019年门诊中药饮片处方情况，旨在规范中药饮片处方，提高处方质量，促进临床合理应用。

1 资料与方法

1.1 一般资料

随机抽取辽宁某医院2017—2019年的中药饮片处方，每个月抽取处方量为200张，共计7200张。
