摘 要：[目的/意義]推进医疗数据共享是“互联网+医疗健康”的关键环节，在隐私保护视角下，构建三方演化博弈模型以分析不同主体的决策行为，探究各方利益相关者的共享意愿。[方法/过程]首先构建了患者、医疗服务机构和政府的三方演化博弈模型，接着分析参与主体在模型中的演化稳定策略，最后探究三方参与主体对医疗数据共享的参与意愿。[结果/结论]基于模型策略均衡和仿真模拟发现，患者参与是推进医疗数据共享的关键因素;政府处罚金额与奖励补贴是影响医疗服务机构策略选择的重要因素，合理的奖惩金额可有效提升患者和医疗服务机构的参与意愿。通过分析投入、收益和成本等因素对隐私保护和医疗数据共享意愿的影响，可提升医疗数据隐私保护水平，推动国内医疗数据共享进程。

Research on Willingness to Share Medical Data from

Perspective of Privacy Protection

——Based on Tripartite Evolutionary Game Analysis

Abstract：[Purpose/Significance]Promoting medical data sharing is a key link of“Internet+medical health”.From the perspective of privacy protection，a tripartite evolutionary game model is constructed to analyze the decision-making behavior of different subjects and explore the sharing willingness of all stakeholders.[Method/Process]First，a tripartite evolutionary game model of patients，medical service institutions and the government was constructed，then the evolutionary stability strategy of the participants in the model was analyzed，and finally the willingness of the tripartite participants to participate in the sharing of medical data was explored.[Result/Conclusion]According to the strategy balance and simulation based on the model，patient participation was a key factor to promote the sharing of medical data.Moreover，the amount of government penalties and rewards and subsidies were important factors that affect the strategic choices of medical service institutions.A reasonable amount of rewards and punishments can effectively increase the willingness of patients and medical service institutions to participate.By analyzing the influence of input，income，cost and other factors on privacy protection and medical data sharing willingness，the level of medical data privacy protection can be improved and the process of domestic medical data sharing can be promoted.

随着物联网和大数据技术的快速发展，各类医疗数据规模呈现出爆发式增长，除了传统的临床医疗数据外，也出现了由智能医疗设备和医疗信息系统生产的各种医疗检查记录。医疗大数据蕴含着非常宝贵的医疗健康信息，近年来成为了各界关注的重点。2018年9月，国家卫健委发布的《关于印发国家健康医疗大数据标准、安全和服务管理办法（试行）的通知》，首次从官方角度给出了医疗数据的定义，即在人们疾病防治、健康管理等过程中产生的与健康医疗相关的数据[1]。……
