【摘 要】目的：分析肿瘤相关性贫血患者，血清铁调素（Hepc）、白细胞介素-6（IL-6）水平的表达及临床意义。方法：选择肿瘤相关性贫血患者36例为观察组，健康者36例为对照组，分析并记录各项检查数据。结果：观察组MCHC、CHR水平均低于对照组，RET水平高于对照组（P<0.05）；观察组患者SI、TSAT數值低于对照组，SF、sTfR数值高于对照组（P<0.05）；两组间Hepc、IL-6、SF水平无显著差异（P>0.05）。结论：肿瘤相关性贫血最主要发病机制为Hepc，是铁代谢中重要的调节激素，可为治疗提供依据。

The expression and clinical significance of serum Hepc and IL-6 levels in patients with tumor-associated anemia

【Abstract】Objective：To analyze the expression and clinical significance of serum hepcidin （Hepc） and interleukin-6 （IL-6） in patients with tumor-related anemia. Methods： Selecting 36 patients with tumor-related anemia as the observation group and 36 healthy subjects as the control group， analyzing and recording various examination data. Results： The MCHC and CHR levels of the observation group were lower than those of the control group， and the RET level was higher than that of the control group（P<0.05）； the values of SI and TSAT of the observation group were lower than those of the control group， and the values of SF and sTfR were higher than those of the control group（P<0.05）； There was no significant difference in the levels of Hepc， IL-6 and SF between the two groups（P>0.05）.Conclusion： The main pathogenesis of tumor-associated anemia is Hepc， which is an important regulatory hormone in iron metabolism， which can provide basis for treatment.

贫血是指外周血中单位容积内红细胞数减少或血红蛋白浓度减低，导致机体不能对周围组织细胞充分供氧的疾病，是肿瘤患者常见的并发症，肿瘤相关性贫血是指肿瘤患者在疾病的发展及治疗过程中发生的贫血，是恶性肿瘤常见的伴随疾病之一，我国肿瘤发病率呈现上升趋势，容易引发多种不良反应，中国肿瘤患者贫血发生率约为60.83%，女性贫血比例高于男性，其中胃癌患者重度贫血的比例最高，患者可出现面色苍白、头晕、乏力、气促等症状，疾病发生原因较多，化疗药物尤其是铂类的使用可促进红细胞的凋亡，引起红细胞计数减少或血红蛋白浓度降低，除此之外，放射治疗及肿瘤的失血、溶血、侵犯骨髓等也会引起贫血，对患者的生活质量及生命安全造成严重影响[1]。疾病主要调节因子为Hepc，Hepc是调节铁凋谢的关键因素，可使血浆中的铁浓度降低，与十二指肠、巨噬细胞结合时，可减少细胞内的铁释放入血浆中，而IL-6等高表达可使 Hepc增多，Hepc与铁蛋白结合，最终导致贫血的发生，本研究分析肿瘤相关性贫血患者血清Hepc、IL-6水平表达及临床意义，现报道如下。……
