摘 要 目的：分析我院门诊中药处方不合理用药情况及对策研究。方法：2017年1月—2018年9月期间随机每月抽取门诊中药处方50张，共1 050张，对其进行分析以及点评。结果：抽取的1 050张处方中，不合理处方共计89张（8.48%），其中辨证不足17张（19.10%）所占比重最大；89张不合理处方中内科18张（20.22%）所占比重最大；中医执业医师开具26张（29.21%），西医执业医师开具63例（70.79%）。结论：根据我院门诊不合理中药处方调查与分析，应定期开展医师尤其是西医医师的理论与实践培训，加强临床合理用药监管力度。

Analysis of irrational use of traditional Chinese medicine prescription in outpatient department of our hospital

ABSTRACT Objective： To analyze the irrational use of traditional Chinese medicine in outpatient department of our hospital and find its countermeasures. Methods： Fifty outpatient Chinese medicine prescriptions were randomly selected every month from January， 2017 to September， 2018 with total 1 050 prescriptions， and they were analyzed and commented. Results： Total 89 unreasonable prescriptions were found from the 1 050 prescriptions， accounting for 8.48%， in which 17 cases （19.10%） of insufficient syndrome differentiation and 18 cases （20.22%） of internal medicine prescriptions accounted for the largest proportion； 26 cases （29.21%） and 63 cases （70.79%） were prescribed by practitioners of Chinese medicine and practitioners of western medicine， respectively. Conclusion： Theoretical and practical training for physicians， especially western medicine physicians， should be regularly carried out and the supervision of clinical rational use of drugs should be strengthened based on this investigation and analysis.

合理用药是指临床医师在用药时做到药物选择正确、用法用量适宜、联合用药合理，尽量降低药物治疗时产生的副作用，让药物的药效起到最佳的治疗效果，能够及时有效地控制患者的病情。中药在中医理论指导下常用于预防、诊断、治疗疾病，由于安全性较高，效果较好，故而在临床上应用广泛。但是由于中药品种多样、配方较多、剂型复杂，在临床上不合理使用情况颇为严重。处方质量直接影响药物疗效，不合理的处方不但浪费药物，甚至会延误以及加重患者病情。加强门诊中药处方的点评和管理能够有助于了解临床用药情况，进而有效规范临床合理用药。本研究对我院门诊中药处方不合理用药情况进行点评，并分析不合理用药的原因，根据原因制定了相应对策，现报道如下。
