[摘要] 目的 分析甲鈷胺联合前列地尔治疗糖尿病周围神经病变的治疗价值。方法 将于2019年1—11月该院收治的102例糖尿病周围神经病变患者作为观察对象，随机分为观察组患者51例，对照组患者51例，对观察组给予甲钴胺联合前列地尔治疗，对照组患者实施前列地尔治疗，对比观察两组患者治疗前后振动感觉阈值、治疗效果、不良反应（面色潮红、食欲不振、腹胀）及患者病变（肢体麻木、灼烧刺痛感、四肢无力）发病次数。结果 治疗前两组患者振动感觉阈值差异无统计学意义（P>0.05），治疗后观察组振动感觉阈值相比于对照组，分数较低且差异有统计学意义（P<0.05），观察组患者治疗有效率与对照组差异有统计学意义（P<0.05），观察组患者不良反应发生率与对照组比较，观察组不良反应发生率更低（P<0.05），观察组肢体麻木、灼烧刺痛感、四肢无力发病次数相比于对照组，差异有统计学意义（P<0.05），观察组治疗后空腹血糖与餐后2 h血糖相比于对照组更低，差异有统计学意义（P<0.05）。结论 应用甲钴胺联合前列地尔治疗糖尿病周围神经病变，能够降低患者的肢体麻木感，提升活动能力，治疗效果显著，值得在治疗糖尿病周围神经病变中应用。

Study on the Therapeutic Value of Mecobalamin Combined with Alprostadil in the Treatment of Diabetic Peripheral Neuropathy

[Abstract] Objective To analyze the therapeutic value of mecobalamin combined with alprostadil in the treatment of diabetic peripheral neuropathy. Methods The patients with diabetic peripheral neuropathy 102 cases admitted to the hospital from January to November 2019 will be randomly divided into observation group of 51 patients， control group of 51 patients， and the observation group will be given mecobalamin combined with alprostadil treatment， the control group of patients received alprostadil treatment， the two groups were compared and observed before and after treatment， the vibration sensory threshold， the treatment effect， adverse reactions（flushing， loss of appetite， abdominal distension） and patient lesions （numbness， burning of the limbs， weakness of limbs）. Results Before treatment， there was no statistically significant difference in vibration perception threshold between the two groups of patients（P>0.05）. After treatment， the vibration perception threshold of the observation group was lower than that of the control group and the difference was statistically significant （P<0.05）. The treatment efficiency of the observation group was compared with the control group， the effective rate was higher （P<0.05）， the incidence of adverse reactions in the observation group was compared with the control group， the incidence of adverse reactions in the observation group was lower（P<0.05）， the observation group had numbness， burning and tingling， and limbs compared with the control group， the number of times of weakness was less and the difference was statistically significant（P<0.05）. The fasting blood glucose after treatment and the 2 h postprandial blood glucose in the observation group were lower than those in the control group， the difference was statistically significant（P<0.05）. Conclusion The application of mecobalamin combined with alprostadil in the treatment of diabetic peripheral neuropathy can reduce the numbness of the patient's limbs， improve mobility， and have a significant therapeutic effect. It is worthy of application in the treatment of diabetic peripheral neuropathy.

糖尿病周围神经病变为常见的慢性疾病，长期患病会引发糖尿病足、微血管病变以及神经性病变等周围神经病变，患者会出现双下肢发凉、麻木及刺痛感，严重损害患者的中枢神经、产生自主神经紊乱，对于糖尿病患者而言，周围神经病变为常见的微血管并发症，发病与微血管产生病变以及患者身体内代谢紊乱存在一定的关系，患者会出现肢体疼痛或麻木，出现足部溃烂等情况，病情严重的会导致患者残疾或截肢，同时病变会对感觉神经以及运动神经产生影响，严重影响患者的生活质量[1-4]。该次将于2019年1—11月该院收治的102例糖尿病周围神经病变患者作为观察对象，分析甲钴胺联合前列地尔治疗糖尿病周围神经病变的治疗价值，现报道如下。
