[摘要] 为建立合理的感染控制措施，医院感染病原菌流行病学调查非常重要。本文阐述了在医院感染领域运用分子流行病学调查方法的进展，并列举了其在医院感染暴发流行病调查中的应用。分子生物学方法主要包括两类：DNA检测技术，如扩增片段长度多态性分析、质粒指纹图谱分析、聚合酶链式反应、脉冲场凝胶电泳技术、DNA测序技术等;蛋白质检测技术，如多位点酶电泳法、免疫印迹技术。各种方法均有优缺点，实际操作中选用何种分型方法，应根据情况来选择。合理的分子流行病学分型方法的使用对医院感染控制工作非常重要。

The progress in research methods in molecular biology of nosocomial infection

[Abstract] The epidemiological investigation of nosocomial infection pathogens is crucial to establish reasonable infection control measures. In this paper， the progress of molecular biological methods in the epidemiological investigation of nosocomial infection pathogens is described， and the application of some molecular biological methods in the epidemiological investigation of nosocomial infection outbreaks is listed. These molecular typing methods can be categorized into two main groups： DNA-based methods， such as amplified fragment length polymorphism， plasmid fingerprinting， polymerase chain reaction， pulsed field gel electrophoresis and DNA sequencing technology; protein-based methods， such as mutilocus enzyme electrophoresis and immunoblotting. Both kinds of methods have advantages and disadvantages. Therefore， which typing method is employed in practice should be selected based on the situation. It is very important for nosocomial infections to use a reasonable molecular biological typing method.

医院感染具有明显的致死与致残率非常高的特点，已经成为住院患者健康及患者预后的最大影响因素。医院感染的发生与各种微生物都有关，其中最为常见的是细菌。近年来感染控制领域难题及焦点是多重耐药菌的感染，为提供有效的感染控制措施，医院感染流行病学调查非常重要，这就需要了解病原菌在医院内的亲缘关系和分布。我们通常采用各种分型技术来确定病原菌的亲缘性，其中传统分型法如细菌素、噬菌体和血清学分型等，这些分型法虽然有用，但容易受多种因素的影响，有很大的变异性，其重复性不佳，工作时间长，工作量也大，不利于医院感染的监控。目前采用核酸技术分析医院感染发生、发展规律，能更加精準高效地完成医院感染的管理控制，已经成为国际医院感染管理研究中的重要方向。……
