【摘 要】目的：探讨自由体位待产对足月胎膜早破孕妇阴道分娩率及产程的影响。方法：选取2017年1月至2018年1月于本院收治的120例足月胎膜早破孕妇，随机分为两组，研究组采取自由体位待产，对照组采取平卧位及交替左右侧卧位的常规体位待产，统计对比两组孕妇的分娩结局。结果：研究组的阴道分娩率高于对照组（P<0.05）;研究组第一产程、第二产程时间均短于对照组（P<0.05），两组第三产程时间、新生儿体质量无明显差异（P>0.05）;研究组产妇产时发热和尿储留的发生率低于对照组（P<0.05），新生儿窒息率、产后出血率、产褥感染率差异无统计学意义（P>0.05）。结论：在研究自由体位待产对足月胎膜早破孕妇阴道分娩率及产程的影响的过程中，发现自由体位待产能够提高孕妇的阴道分娩率，有效缩短第一产程时间和第二产程时间，自由体位待产值得推广应用。

Effect of free position on the vaginal delivery rate and labor process of pregnant women with premature rupture of membranes

[Abstract] Objective：To investigate the effect of free position on the vaginal delivery rate and labor process of pregnant women with premature rupture of membranes. Methods： A total of 120 pregnant women with premature rupture of membranes were enrolled in our hospital from January to February 2018， and were randomly divided into two groups. The study group took a free position and the control group took a supine position and alternated left and right sides. The regular position of the position is expected to be produced， and the outcome of the delivery of the two groups of pregnant women is statistically compared. Results： The vaginal delivery rate of the study group was higher than that of the control group （P<0.05）. The first stage of labor and the second stage of labor in the study group were shorter than the control group （P<0.05）. The third stage of labor and the weight of the newborn were not in the two groups. The difference was significant （P>0.05）. The incidence of gynecological fever and urine retention in the study group was lower than that in the control group （P<0.05）. There was no significant difference in neonatal asphyxia rate， postpartum hemorrhage rate and puerperal infection rate. （P>0.05）. Conclusion： In the study of the effect of free position on the vaginal delivery rate and labor process of pregnant women with premature rupture of membranes， it is found that free position to improve the vaginal delivery rate of pregnant women， effectively shorten the first stage of labor and the second stage of labor， It is recommended that the free position be promoted and applied.

臨产前胎膜自然破裂称之为胎膜早破。妊娠达到及超过孕37周发生者称足月胎膜早破。足月单胎妊娠胎膜早破发生率为8%，是产科常见并发症之一，是难产信号，其增加了孕产妇感染的机会和剖宫产率[1]。对于足月头位已衔接胎膜早破孕妇的待产体位，目前尚无明确的要求。本研究主要探讨自由体位待产对足月胎膜早破孕妇阴道分娩率及产程的影响，现报道如下。

1 資料与方法

1.1 一般资料

选取2017年1月至2018年1月本院收治的足月胎膜早破孕妇120例，随机分为两组。研究组孕妇60例，年龄25～34岁，平均年龄（29.6±1.4）岁，孕周37～41周。对照组孕妇60例，年龄25～35岁，平均年龄（29.5±1.2）岁，孕周37～40周。两组产妇的一般资料有可比性（P>0.05），均知情同意本研究，并经过医院伦理委员会批准。
