【摘要】 目的：探讨舒适护理对腹腔镜下直肠癌根治术后吻合口瘘患者的应用效果。方法：选取笔者所在医院2016年1月-2019年1月收治的80例腹腔镜直肠癌根治术后并发吻合口瘘患者作为研究对象，按照随机数字表法分为常规护理组（40例）与舒适护理组（40例）。常规护理组采用常规护理方法，舒适护理组采用舒适护理方法。比较两组患者SAS評分、SDS评分、住院时间及住院费用。结果：护理2周后及出院时，舒适护理组SAS、SDS评分均明显低于常规护理组，差异有统计学意义（P<0.05）。舒适护理组住院时间短于常规护理组，住院费用低于常规护理组，差异均有统计学意义（P<0.05）。结论：对腹腔镜直肠癌根治术后吻合口瘘患者施行舒适护理可以有效降低患者焦虑、抑郁情绪，缩短患者住院时间，减轻其经济负担。

【Abstract】 Objective：To explore the application effect of comfortable nursing for patients with anastomotic fistula after laparoscopic radical resection of rectal cancer.Method：A total of 80 patients with anastomotic fistula after laparoscopic radical resection of rectal cancer admitted in our hospital from January 2016 to January 2019 were selected as the research objects.They were divided into the routine nursing group（40 cases） and the comfortable nursing group（40 cases） according to random number table method.The routine nursing group adopted the routine nursing method，and the comfortable nursing group adopted comfortable nursing method.The SAS scores，SDS scores，hospitalization time and hospitalization cost of the two groups were compared.Result：The scores of SAS and SDS in the comfortable nursing group were significantly lower than those in the routine nursing group after 2 weeks of nursing and at discharge，the differences were statistically significant（P<0.05）.The hospitalization time of the comfortable nursing group was shorter than that of the routine nursing group，and the hospitalization cost was lower than that of the routine nursing group，the differences were statistically significant（P<0.05）.Conclusion：Comfortable nursing for patients with anastomotic fistula after laparoscopic radical resection of rectal cancer can effectively reduce patients anxiety and depression，shorten the hospitalization time，and reduce their economic burden.

直肠癌是消化道常见的恶性肿瘤之一，其发病率位于我国恶性肿瘤第3位[1]，占结直肠癌患者的2/3，主要的治疗手段为手术切除为主的综合治疗。腹腔镜直肠癌根治术具有术后出血少、疼痛小、恢复快、切口小、进食早、下床活动早等优点[2]，患者术后保肛几率明显增加，但目前吻合口瘘仍是该术式常见的并发症之一[3]，其不利于伤口愈合、延长患者住院时间、增加治疗费用，且对患者的身心产生消极影响。笔者所在医院护理人员长期致力于改良直肠癌根治术后吻合口瘘的护理措施。本研究对舒适护理在直肠癌根治术后吻合口瘘患者中的效果进行探讨，现报告如下。

1 资料与方法

1.1 一般资料

选取笔者所在医院2016年1月-2019年1月收治的80例腹腔镜直肠癌根治术后吻合口瘘患者作为研究对象，纳入标准：（1）经病理确诊为直肠癌，且无远处转移;（2）符合直肠癌根治术手术指征;（3）术后出现吻合口瘘;（4）无神经精神类疾病，可配合完成研究。……
