NSCLC患者血清miR系列基因表达水平及与化疗疗效的相关性研究

[摘要] 目的 探討非小细胞肺癌(non-small lung cancer, NSCLC)患者血清miR (microRNA)系列基因表达水平及与化疗疗效的相关性。 方法 2011年3月~2016年9月选择200例未经任何抗癌治疗的NSCLC患者作为NSCLC组,100例健康志愿者作为健康组,分别收集血清样本;25例晚期NSCLC患者化疗前和化疗后的血清样本。采用qRT-PCR方法检测NSCLC组和健康组中血清miR系列基因表达水平,判断血清miR系列基因表达水平与预后的关系。 结果 NSCLC组的血清miR-19a、miR-19b-1、miR-20a、miR-17-3p、miR-18a和miR-92-1基因表达与健康组存在差异(P<0.01)。NSCLC患者的血清上述基因表达化疗前与化疗后存在明显差异(P<0.05);健康组的血清miR-19a、miR-19b-1、miR-20a、miR-17-3p、miR-18a和miR-92-1基因表达与化疗前患者存在明显差异(P<0.05),与化疗后的患者比较,差异无统计学意义(P>0.05)。 结论 NSCLC患者血清存在高水平miR系列基因表达,化疗可降低miR系列基因表达水平,上述6种miR基因血清表达有助于NSCLC的诊断、预测和预后的判断,联合检测有助于敏感性的提高。

Study on the correlation between the expression level of serum miR gene and chemotherapy efficacy in NSCLC patients

[Abstract] Objective To investigate the correlation between the expression levels of serum miR(microRNA) gene and chemotherapy efficacy in the patients with non-small cell lung cancer(NSCLC). Methods From March 2011 to September 2016, 200 NSCLC patients without any anticancer treatment were selected as the NSCLC group. 100 healthy volunteers were selected as the healthy group. Serum samples were collected separately; serum samples before and after chemotherapy in 25 patients with advanced NSCLC were collected. The qRT-PCR method was used to detect the expression level of serum miR gene in NSCLC group and healthy group, and the relationship between the expression level of serum miR gene and prognosis was determined. Results The serum levels of miR-19a, miR-19b-1, miR-20a, miR-17-3p, miR-18a, and miR-92-1 in the NSCLC group were different from those in the healthy group(P<0.01). There were significant differences in the expressions of the above genes before and after chemotherapy in the serum of NSCLC patients(P<0.05); the serum levels of miR-19a, miR-19b-1, miR-20a, miR-17-3p, miR-18a and miR-92-1 in the healthy group were significantly different from those before chemotherapy(P<0.05). There was no statistical difference compared with the patients after chemotherapy(P>0.05). Conclusion There is a high level of miR gene expression in the serum of NSCLC patients. Chemotherapy reduces miR gene expression levels. The above six miR gene serum expressions contribute to the diagnosis, prediction and prognosis of NSCLC. The joint detection helps increase sensitivity.

中国恶性肿瘤中发病率和病死率最高的是肺癌,而且非小细胞肺癌(NSCLC)占85%,常见临床分期是ⅢB或Ⅳ期。化疗依然是晚期NSCLC不可替代的手段,其治疗目标是延长患者总生存期(OS)、提高生活质量(QOL)、降低疾病相关的并发症。虽然,针对肺癌的化学预防和治疗已经取得了一些进展,但肺癌治疗效果依旧不佳,近年来的研究表明肺癌的发生与转移是一个极其复杂的多基因调控的多阶段、多步骤过程。因此,从基因表达调控方面研究肺癌发生发展的机制,了解肺癌治疗预测的相关因素,能更精准预测化疗疗效,优化治疗方案,提高肺癌患者的生活质量和远期生存率已经十分必要。microRNA(miRNA)是一类具有在转录后水平调控基因表达功能的非编码小分子miRNA,在细胞的生长、增殖、代谢、凋亡等方面都有着重要的作用。……
