【摘要】 目的：探究不同剂量舒芬太尼复合左布比卡因蛛网膜下腔阻滞在前列腺电切术中的可行性及临床应用价值。方法：选择2013年10月-2016年11月收治的采用腰硬联合麻醉下行前列腺电切术的80例老年患者，ASAⅠ～Ⅲ级，随机将其分为四组，每组20例，A组单用左布比卡因7.5 mg，B组左布比卡因5 mg+舒芬太尼2 μg，C组左布比卡因5 mg+舒芬太尼3 μg，D组左布比卡因5 mg+舒芬太尼4 μg。观察四组患者临床麻醉效果；记录四组患者不良事件发生率。结果：T1时，B组、C组、D组SBP、DBP、HR与A组数值相比，差异有统计学意义（P<0.05）；T1、T2时A组、B组、C组及D组患者的SBP、DBP与T0时数值比较，差异有统计学意义（P<0.05）；D组发生不良事件发生率高于A组、B组、C组，差异有统计学意义（P<0.05）。结论：在本试验条件下左布比卡因5 mg复合舒芬太尼2 μg或3 μg用于蛛网膜下腔阻滞前列腺电切术的老年患者血流动力学平稳，效果较佳，且不良事件较少。

【Abstract】 Objective：To observe the effecacy of taking different doses of Sufentanil combined Levobupivacaine subarachnoid block clinical application value in elderly patients with transurethral resection of prostate.Method：From October 2013 to November 2016，eighty ASA Ⅰ to Ⅲ elderly patients scheduled for transurethral resection of prostate under subarachnoid block anesthesia were equally randomized into four groups，A group was used Levobupivacaine 7.5 mg alone，B group was Levobupivacaine 5 mg and Sufentanil 2 μg，C group was Levobupivacaine 5 mg and Sufentanil 3 μg，D group was Levobupivacaine 5 mg and Sufentanil 4 μg，clinical anesthesia effects，side effects after surgery were also recorded in four groups.Result：At T1，SBP，DBP and HR were lower in A group than B group，C group and D group（P<0.05）.At T1 and T2，SBP and DBP were lower in A group，B group，C group and D group than T0（P<0.05），A group，B group and C group had lower incidence of side effects than D group（P<0.05）.Conclusion：Levobupivacaine 5 mg combined with Sufentanil 2 μg or 3 μg subarachnoid block in elderly patients with transurethral resection of prostate can make blood flow dynamics remain stable with less side effects.

前列腺增生是老年男性常見的一种疾病，随着我国老龄化的增快，该疾病的发病率呈上升的趋势。对于该疾病的手术治疗以前列腺电切术为主，为探究不同剂量舒分太尼（批号：1140320，国药准字H20054171，宜昌人福药业）复合盐酸左布比卡因（批号：150109CA，国药准字H20020570，江苏恒瑞医药）在前列腺电切术中的作用效果，现将笔者所在医院自2013年10月-2016年11月收治的80例患者作为研究对象，做如下报道。

1 资料与方法

1.1 一般资料

本研究经南京市溧水区人民医院伦理委员会批准同意，患者签署知情同意书后，在南京市溧水区人民医院麻醉科实施开展。选择2013年10月-2016年11月收治的采用腰硬联合麻醉下行前列腺电切术的80例老年患者，ASAⅠ～Ⅲ级，年龄65～80岁，体重49～86 kg，身高152～183 cm。既往无明显的心、肝、肺、脑、肾等系统疾病，无凝血功能异常，无阿片类药物过敏史。排除标准：有神经、精神及内分泌系统疾病患者；……
