【摘要】 目的：探究保留膀胱功能髂内动脉化疗治疗膀胱癌的临床效果。方法：选择2012年1月-2014年9月笔者所在医院收治的行保留膀胱手术的膀胱癌患者80例为研究对象，根据治疗方法的差异分为对照组与观察组，对照组给予常规膀胱灌注化疗，观察组给予髂内动脉化疗，比较两组治疗效果。结果：观察组治疗2年后生存率显著高于对照组，复发及死亡率显著低于对照组，差异有统计学意义（P<0.05）；两组患者治疗过程中发生肠道反应、腹部不适、白细胞减少、血小板减少率差异无统计学意义（P>0.05）。结论：保留膀胱功能髂内动脉化疗治疗膀胱癌效果显著，且治疗过程中不良反应小，有效提高了患者的生存期及生活质量。

【Abstract】 Objective：To explore the clinical effect of retaining bladder functional iliac artery chemotherapy for bladder cancer.Method：From January 2012 to September 2014，in our hospital lines retain bladder surgery of 80 cases of bladder cancer patients were selected as the research object，according to the differences in treatment were divided into the control group and the observation group，the control group given conventional bladder perfusion chemotherapy，in the observation group was given iliac arterial chemotherapy，two groups of therapeutic effect were compared.Result：The survival rate in the observation group was significantly higher than that in the control group after 2 years，and the risk of relapse and death was significantly lower in the observation group than that in the control group，and the difference was statistically significant（P<0.05）.There was no significant difference in the incidence of intestinal reaction，abdominal discomfort，leukocyte reduction，and thrombocytopenia in the two groups（P>0.05）.Conclusion：It is effective to retain bladder functional iliac artery chemotherapy to treat bladder cancer，and it can improve the survival and quality of life.

膀胱癌是泌尿外科中發病率最高的一种恶性肿瘤，该肿瘤通常从盆腔淋巴结开始，经淋巴结转移，同时具备复发与浸润的生物学特征[1]。既往对膀胱癌的治疗主要为根治性膀胱癌切除联合盆腔淋巴结清扫术，国外数据表明，通过近20年的数据统计，效果并不满意，膀胱癌患者无明显生存周期延长[2]。近年来临床对于膀胱癌的治疗更趋向保留患者的膀胱功能，治疗方法转化为尽可能地保留膀胱功能并辅以化疗等综合手段[3]。在保留膀胱部分功能，提高生活质量的同时，还能提高患者的生存率，保留膀胱功能的手术避免了患者术后出现尿失禁或勃起性功能障碍等，是目前医学界公认的治疗膀胱癌的可靠手段[4]。膀胱癌在行保留膀胱手术治疗后，常使用膀胱灌注化疗与髂内动脉化疗，现对既往收治的80例患者行回顾性分析，现将其做如下报道。……
