【摘要】 目的：探讨早期乳腺癌分子亚型与腋窝前哨淋巴结状态的相关性。方法：根据2013年St. Gallen国际乳腺癌共识，将264例早期乳腺癌患者分为Luminal A、Luminal B（HER2+）、Luminal B（HER2-）、HER-2 过表达、basal-like五个分子亚型，其中前哨淋巴结阳性59例，比较不同分子亚型间腋窝前哨淋巴结转移率的差异。结果：Luminal B-型转移率较高，Luminal A型和HER-2过表达型次之，不同分子亚型乳腺癌的腋窝前哨淋巴结转移率比较差异有统计学意义（P=0.046）。结论：不同分子亚型乳腺癌腋窝前哨淋巴结转移特点不同。分子分型是对传统病理分类的重要补充，可以更好地反应乳腺癌的生物学行为。

【Abstract】 Objective：To investigate the association between molecular subtypes of early breast cancer and sentinel lymph node status. Method：264 cases of early breast cancer were divided into Luminal A， Luminal B （HER2+），Luminal B （HER2-），HER2 overexpression， and basal-like five molecular subtypes according to the St Gallen International Breast Cancer Conforence（2013）.The rate of axillary sentinel lymph node metastases（SLNM） was compared among the five groups. Result：The rate of SLNM was the highest in patients with Luminal B-like （HER2-negative） breast cancer， and the rates of Luminal A-like and HER-2 overexpression followed. The rate of SLNM was significantly different among the five groups （P=0.046）.Conclusion：There characteristics of SLNM was different among molecular subtypes of breast cancer. Molecular type is an important complement to traditional pathological classification， which can better imply the biological behavior of breast cancer.

乳腺癌前哨淋巴结活检（sentinel lymph node biopsy， SLNB）是近年来乳腺外科治疗领域中热点，提供了以最小损伤来获取乳腺癌患者腋窝淋巴结病理状态的方法。乳腺癌的分子生物学特性与其组织病理学紧密相关，进而决定了其侵袭能力的差异性。本研究将早期乳腺癌前哨淋巴结状态与乳腺癌患者分子亚型进行对比分析，探讨其相关性。

1 资料与方法

1.1 一般资料

选取2008年1月1日-2014年12月31日在笔者所在医院乳腺中心行SLNB的264例女性乳腺癌患者（共271例患者行SLNB，其中7例未显影，另外264例纳入分析）。排除内乳前哨淋巴结转移（Sentinel Lymph Node Metastasis，SLNM）和乳腺肉瘤。患者年龄（52.1±10.0）岁，体质指数为（25.9±3.6）kg/m2，平均切除前哨淋巴结2.74枚。264例研究对象中，前哨淋巴结阳性59例，转移率22.3%，均行腋窝淋巴结清扫术，前哨淋巴结阴性 205例，未行腋窝清扫。左侧乳腺癌118例，右侧143例，双侧3例。……
