摘 要 目的：通过分析浦兴社区卫生服务中心门诊患者阿仑膦酸钠的使用状况，旨在促进中心临床合理用药。方法：收集并统计2016年1—12月浦兴社区卫生服务中心所有门诊患者的阿仑膦酸钠处方，共343张，分析患者的用药合理性。结果：60岁以上老年患者是阿仑膦酸钠的主要用药人群；每周一次口服使用为患者的主要服药方法；有适应证用药率为97.37%，配伍使用率为83.09%；超说明书用药主要表现为适用人群、用法用量以及适应证方面，分别有2例，49例和146例。结论：社区卫生服务中心应加强处方点评工作，干预不合理的超说明书用药现象，促进临床合理用药。

Analysis of clinical application of alendronate sodium in the community health service center

ABSTRACT Objective： To analyze the utilization status of alendronate sodium in the patients in the outpatient clinic of Puxing Community Health Service Center， so as to promote clinical rational drug use in the center. Methods： A total of 343 prescriptions of alendronate sodium in the patients in Puxing Community Health Service Center were collected and counted from Jan. to Dec. 2016 to analyze the patients medication rationality. Results： Elderly patients aged 60 or older were the main drug users for alendronate sodium， weekly oral use was the main method of medication for the patient， medication rate was 97.37% with indications， compatible use rate was 83.09%， off-label drug use was mainly for the applicable crowd， and the usage， dosage and indication were 2， 49 and 146 cases， respectively. Conclusion： The community health service centre should strengthen the prescription comment and interfere with the irrational off-label drug use in order to promote rational use of drugs in the clinic.

阿仑膦酸钠属第3代氨基二膦酸盐类骨吸收抑制剂[1]，是首个获得美国食品药品监督管理局批准用于抗骨质疏松治疗的双膦酸盐类药物。阿仑膦酸钠通过增加骨密度降低患者髋部骨折风险，适用于骨关节炎、畸形性骨炎，特别是治疗恶性肿瘤骨转移引起的骨骼相关事件，疗效显著[2]。由于阿仑膦酸钠在临床应用广泛，其用药合理性也随之成为了临床重视的问题。本文就浦东新区浦兴社区卫生服务中心2016年阿仑膦酸钠的使用情况进行统计、分析，为社区卫生服务中心合理用药提供参考。

1 资料与方法

1.1 资料收集

收集、整理浦兴社区卫生服务中心HIS系统中2016年度1—12月门诊患者中使用阿仑膦酸钠肠溶片（70 mg×1片/盒）的处方信息，共收集处方343张。……
