[摘要] 目的 探讨基于全科医护团队的中医健康管理模式在改善老年慢性疾病患者满意度中的应用效果。 方法 选取2016年6～12月本中心慢病门诊就诊的200例老年慢病患者随机分为管理组和参照组，管理组采取全科医护团队中医健康管理模式，参照组进行常规随访，比较干预后生活质量、满意度。 结果 管理组健康、情感、认知、生活、工作、生理评分均高于参照组（P<0.05）。管理组患者满意度高于参照组（P<0.05）。 结论 基于全科医护团队的中医健康管理模式在老年慢性病患者中应用可提高患者生活质量和满意度，有显著临床实施意义。

Evaluation of application effect of TCM health management model based on general medicine health care team in improving satisfaction of elderly patients with chronic diseases

[Abstract] Objective To explore application effect of TCM health management model based on general medicine health care team in improving satisfaction of elderly patients with chronic diseases. Methods 200 elderly patients with chronic diseases who were registered in outpatient service from June 2016 to December 2016 were selected and they were randomly divided into the management group and the reference group. The management group was given TCM health management model based on general medicine health care team while the reference group was given routine follow-up. Quality of life and satisfaction after intervention were compared. Results Scores of health， emotion， cognition， life， work and physiology of the management group were all higher than those of the reference group （P<0.05）. The satisfaction rate of the management group was higher than that of the reference group （P<0.05）. Conclusion Application of TCM health management model based on general medicine health care team in elderly patients with chronic diseases can improve quality of life and satisfaction of patients， which has significant clinical significance of implementation.

随着我国人口老年化，老年慢性疾病在临床中占有比例显著升高，为影响老年患者健康和生活质量主要影响因素[1-2]。临床调查显示，高血压、糖尿病、代谢综合征及心、肺等慢性疾病均好发于老年患者，且在老年人群中具有较高发病率[3-5]。健康管理为延缓慢性疾病发生、发展，降低相关并发症发生关键[6]。为探究基于全科医护团队的中医健康管理模式在老年慢性疾病患者中实施对患者生活质量及满意度影响效果，笔者研究如下。

1 资料与方法

1.1 一般资料

选取2016年6～12月本中心慢病门诊就诊

的200例老年慢病患者进行研究，将信息录入计算机中随机分为管理组和参照组，每组100例。管理组中男56例，女44例；年龄65～88岁，平均（73.8±5.3）岁；合并症：高血压病44例，糖尿病43例，高脂血症46例，冠心病39例；病程6月～22年，平均（7.3±2.1）年。參照组中男54例，女46例；年龄65～89岁，平均（73.5±5.9）岁；合并症：高血压病48例，糖尿病42例，高脂血症45例，冠心病36例；病程6个月～23年，平均（7.4±2.2）年。……
