[摘要] 目的 系统评价水针疗法用于初产妇自然分娩镇痛的临床效果。 方法 计算机检索中国期刊全文数据库（CNKI）、万方数据知识服务平台、中国维普全文数据库（VIP）、Cochrane Library、PubMed、Medline，检索时间从建库～2016年8月，收集有关水针疗法用于分娩镇痛的随机对照试验，进行严格的质量评价后采用RevMan5.3软件进行Meta分析。 结果 共纳入12篇随机对照试验文献，Meta分析结果证实，水针疗法临床镇痛效果好（Z=21.25，P<0.01），并且可以缩短产程（Z=29.56，P<0.01），有效降低剖宫产率（Z=5.0，P<0.01），减少产后出血（Z=28.59，P<0.01），不增加新生儿窒息（Z=1.06，P<0.01）。 结论 现有证据显示，水针疗法用于分娩镇痛效果好，能缩短产程，降低剖宫产率，减少产后出血，对母婴无不良影响。且经济、操作简单、创伤小，且未额外增加临床工作量，适合在基层医院推广应用。

Meta-analysis on hydro-acupuncture therapy for labor analgesia of primipara

[Abstract] Objective To systematically evaluate the clinical effect of hydro-acupuncture therapy for spontaneous labor analgesia of primipara. Methods The data of China National Knowledge Infrastructure （CNKI），Wanfang Data Knowledge Service Platform，VIP，Cochrane Library，PubMed and Medline from creating database to August，2016 were searched.Randomized controlled trials of hydro-acupuncture therapy for labor analgesia were collected and they were given meta-analysis by using RevMan 5.3 software after a strict quality evaluation. Results A total of 12 randomized control trials were included.Meta-analysis results confirmed that clinical analgesic effect of hydro-acupuncture therapy was good （Z=21.25，P<0.01）.In addition，it can shorten the birth process （Z=29.56，P<0.01），effectively reduce the rate of cesarean section （Z=5.0，P<0.01）， reduce postpartum hemorrhage （Z=28.59，P<0.01） and increase no neonatal asphyxia （Z=1.06，P=0.29）. Conclusion Present evidence shows hydro-acupuncture therapy for spontaneous labor analgesia has a good effect.In addition，it can shorten the birth process，reduce the rate of cesarean section，reduce postpartum hemorrhage and be of no harmful effect on mothers and infants.It is an economical，easy-operation and little traumatic method and it won't increase the clinical workload，worthy of promotion and application in primary hospitals.

隨着人性化助产服务理念的发展，以及孕妇对舒适安全分娩需求的日益增加，加之二胎政策的全面放开带来产科医护人员相对不足，寻求一种简单、易行且不影响母婴健康并能减轻分娩疼痛的方法成为产科工作者探讨的课题。国内外有效的分娩减痛方法有多种，包括硬膜外自控镇痛（PECA）等药物镇痛和水中分娩等非药物镇痛，但常因设备及操作繁琐、镇痛必须有麻醉师等其他人的参与、镇痛药物毒副作用大等原因而限制了其推广应用[1]。水针疗法是将无菌注射用水注入皮内起到机械性强刺激及压迫作用，可阻断部分神经传导，促进体内β-内啡肽的升高，提高产妇的疼痛阈值，达到镇痛的目的，有大量临床研究将水针疗法用于分娩镇痛治疗。本研究旨在通过对水针疗法用于初产妇自然分娩镇痛的临床效果进行系统评价，现报道如下。

1 资料与方法

1.1 文献纳入标准与排除标准
