【摘要】目的觀察枸地氯雷他定与自血疗法联合治疗自身免疫性荨麻疹的临床疗效。

方法选取218例自体血清皮肤试验阳性的慢性荨麻疹患者为研究对象，随机分成对照组、观察1组和观察2组，对照组口服枸地氯雷他定片治疗，观察1组给予自血疗法，即抽取患者自身静脉血肌注，1次/周，观察2组在自血疗法的同时口服枸地氯雷他定片治疗，三组患者均治疗8周；记录治疗前后瘙痒程度、风团数量等（UAS评定、4级评定法）变化情况，对比分析三组患者的疗效。

结果治疗8周后，对照组总有效率为74.29%，观察1组总有效率为83.33%，观察2组总有效率为92.11%，两个观察组的临床症状改善效果均明显优于对照组，差异有统计学意义（P<0.05），观察2组与对照组间疗效比较差异有统计学意义（P<0.01）。

结论自身免疫性荨麻疹采用枸地氯雷他定与自血疗法联合治疗，能显著改善患者的临床症状。

An efficacy evaluation on desloratadine citrate combined with autohemotherapy in the treatment of autoimmune urticaria

【Abstract】ObjectiveTo evaluate efficacy of desloratadine citrate combined with autohemotherapy in the treatment of autoimmune urticaria.

Methods218 cases of chronic urticaria patients with positive autologous serum skin test （ASST） were selected as study subjects.All patients were randomly classified into observation group one，observation group two and control group.The control group were treated with oral desloratadine citrate tablets，the observation group one were given autohemotherapy，that was to intramuscularly inject patientsown venous blood once a week，and the observation group two were given oral desloratadine citrate tablets in the basis of autohemotherapy.All groups were treated for 8 weeks.Then，changes of itch degree and wheal numbers（UAS evaluation and 4 grade evaluation method） before and after treatment were recorded，and efficacy of the three groups were compared and analyzed.

ResultsAfter 8 weeks of treatment，total efficacy of the control group was 74.29%，the observation group one was 83.33%，and the observation group two was 92.11%，improvement of clinical symptoms of the two observation groups was all significantly better than that of the control group，difference was statistically significant（P<0.05）.In addition，there was statistically significant difference in curative effects between the observation group two and the control group（P<0.01）.

ConclusionDesloratadine citrate combined with autohemotherapy in the treatment of autoimmune urticaria can significantly improve patients clinical symptoms.

荨麻疹是皮肤科的常见病、多发病，临床上表现为风团伴剧烈瘙痒，慢性荨麻疹（CU）是指风团反复发作，每周至少2次，每次不超过24 h，并持续6周以上[1]。自身免疫性荨麻疹是近年来逐渐被人们认识的CU的一种特殊类型，目前其临床诊断主要靠自体血清皮肤试验（ASST）[2]，在CU患者中，ASST阳性提示患者为自身免疫性荨麻疹。研究表明自身免疫性荨麻疹占CU的比例高达50%～60%[3]，很难控制和治愈，病程迁延，治疗较棘手，增加了患者的痛苦。……
