醒脑静注射液联合新鲜冰冻血浆治疗脓毒症相关性脑病患儿的效果

目的观察醒脑静注射液联合新鲜冰冻血浆对脓毒症相关性脑病（SAE）患儿的临床疗效。方法将2015年1月～2016年1月在广州市番禺中心医院儿科重症监护病房住院的40例SAE患儿按照随机数字表法分成实验组和对照组，每组各20例。对照组给予抗感染、改善微循环、纠正代谢紊乱等基本治疗，实验组在此基础上联合醒脑静注射液和新鲜冰冻血浆静脉注射治疗。观察两组患儿的感染指标、退热时间、抽搐次数、住院时间、神经系统症状缓解率。结果治疗7 d后，两组SAE患儿的白细胞（WBC）、C反应蛋白（CRP）、降钙素原（PCT）水平较治疗前均明显降低，但实验组与对照组比较，WBC、CRP、PCT水平下降更显著，差异均有统计学意义（P＜0.05）。实验组患儿的退热时间、抽搐次数、住院时间明显少于对照组，差异有统计学意义（P＜0.05）。实验组患儿的神经系统相关症状缓解率达到85%，明显高于对照组（55%），差异有统计学意义（P＜0.05）。结论应用醒脑静注射液和新鲜冰冻血浆对SAE患儿有较好的临床效果。

［Abstract］Objective To observe the efficacy of Xingnaojing Injection and fresh frozen plasma in treatment of the children with sepsis associated encephalopathy（SAE）．Methods Totally 40 children with SAE from January 2015 to January 2016 at PICU of Panyu District Central Hospital of Guangzhou were randomly assigned into the control group（20 cases）and the treatment group（20 cases）according to the random number table．Patients in the control group received anti-infection and improving microcirculation，while those in the treatment group were intravenously injected with Xingnaojing Injection and fresh frozen plasma.The infection index，fever clearance time，convulsion time，length of stay，remission rate of nervous system symptom were observed in the two groups．Results After 7 days of treatment，the levels of WBC，CRP，PCT of two groups were obviously decreased，but compared with the control group，the levels of WBC，CRP，PCT in the treatment group were obviously decreased，the differences were statistically significant（P< 0.05）.The fever clearance time，convulsion time and length of stay in the treatment group were obviously decreased comparing with the control group，the differences were statistically significant（P<0.05）.The remission rate of nervous system symptom in the treatment group was 85%，obviously improved comparied with the control group（55%），the difference was statistically significant（P<0.05）.Conclusion Xingnaojing Injection and fresh frozen plasma were effective in treatment of the children with SAE．

脓毒症相关性脑病（sepsis associated encephalopathy，SAE）是脓毒症引起的弥漫性脑功能障碍，严重威胁着患者的生命安全，并常使幸存者遗留不同程度的认知障碍，最终影响患者的生存质量［1］。目前SAE没有特殊的治疗方法，治疗的重点是病因治疗和对症治疗，比如控制脓毒症、纠正器官功能衰竭和代谢紊乱、避免使用神经毒性药物等。……
