预留桩道对纤维桩固位影响的临床研究

目的 通过临床观察探讨根充时预留桩道，两周后桩道预备的可行性。方法 整群选择2012年8—10月在北京市顺义区医院口腔科就诊的前牙缺损患者96颗前牙，将其随机平均分成2组,每组48颗：Ⅰ组：根充时预留桩道，两周后桩道预备纤维桩修复；Ⅱ组：根管常规热牙胶垂直加压充填，两周后桩道预备，纤维桩修复。随访两年，观察固位情况。结果Ⅰ组2颗患牙纤维桩脱落，成功率为96%；Ⅱ组3颗患牙纤维桩脱落，成功率为94%。两组治疗成功率比较差异无统计学意义（P＞0.05）。结论 预留桩道两周后桩道预备具有良好的临床应用价值。

［Abstract］Objective To study the clinical effects of reserved post space during the root canal filling？and post space preparation was performed two weeks later.Methods Group selection a total of 96 anterior tooth in the hospital of Shunyi District Beijing from August to October in 2012 were collected.They were randomly divided into two groups equally.Ⅰgroup：Reserve the post space during root canal filling and two weeks later post space preparation was performed.Ⅱgroup：Post space preparation were performed 2 weeks later after normal root canal filling.Follow-up for 2 years to observe the fiber post retention.Results Two teeth failed in GroupⅠ,three in GroupⅡ.Group I had succeed rate 96%，GroupⅡ94%. There is no significant difference of bond strength between groupⅠandⅡ.Conclusion There is some clinical and practical value in reserved post space during the root canal filling and post space preparation was performed two weeks later.

近年来，根管治疗采用热牙胶垂直加压充填技术越来越受到口腔医师的青睐。对于牙体缺损严重，根管治疗后需桩冠修复的患牙，根管充填时预留桩道可简化根管充填和桩道预备两步临床操作，减少患者就诊次数避免了在操作中烫伤患者口角、颊粘膜等组织，无需取出部分牙胶这一过程，桩道预备时视野更好，减少侧穿发生率，减少了对已充填牙胶的影响。但预留桩道是否影响桩核固位鲜有报道，该文拟对比研究预留桩道对纤维桩固位力的影响。整群选择2012年8月—10月在北京市顺义区医院口腔科就诊的96例前牙缺损患者。并追踪观察两年，检查修复体是否有松动脱落，牙体折断或疼痛等异常，以期指导临床治疗工作。……
