水针刀联合颈2椎旁注射治疗颈源性头痛疗效探讨

目的 探讨水针刀联合颈2椎旁注射治疗颈源性头痛的临床疗效。方法 方便选取该院于2013年6月—2014 年4月收治的58例颈源性头痛患者，随机分为两组：对照组29例采用颈2椎旁阻滞法；观察组29例除采用颈2椎旁阻滞外，配合水针刀局部注射治疗。记录两组患者治疗前及治疗后1周、2周、3周、1个月、3个月的VAS疼痛评分、颈椎活动度（ROM）及患者疗效满意度评价。结果观察组治疗后1周、2周、3周、1个月、3个月的VAS疼痛评分分别为（2.0±1.3）、（1.5±0.9）、（1.8±0.8）、（2.3±0.7）、（2.2±1.1）下降较对照组明显（P＜0.05），且治疗后1周、2周的颈椎活动度（ROM）优于对照组，患者疗效满意度评价观察组（79.31%）优于对照组（65.52%，P＜0.05），差异有统计学意义。结论水针刀联合颈2椎旁注射治疗颈源性头痛可取得较满意临床疗效。

［Abstract］Objective To investigate the effects of water acupotomy combined with C2 paravertebral nerve block on Cervicogenic Headache.Methods Convenient select the hospital from June 2013 to Apirt 2014 years 4 months from 58 patients with cervicogenic headache patients were randomly divided into two groups：the control group of 29 cases with cervical 2 paravertebral block；observation group（29 cases）in addition to the cervical 2 paravertebral block,with local injection of acupotomy therapy.1 groups of patients before treatment and two weeks,2 weeks,3 weeks,weeks,January,March,VAS pain score,cervical activity（ROM）and patient satisfaction evaluation.Results Observation of treatment group after 1 week, 2 weeks,3 weeks,1 month and 3 months of VAS pain scores respectively（2.0±1.3）,（1.5±0.9）,（1.8±0.8）,（2.3±0.7）,（2.2± 1.1）decreased significantly compared with the control group（P＜0.05）,and 1 week and 2 weeks after treatment of cervical live motion（ROM）was better than the control group,patients satisfaction evaluation observation group（79.31%）than in the control group（65.52%,P＜0.05）.the difference was seatisrrically significant.Conclusion The effect of water acupotomy combined with C2 paravertebral nerve block on Cervicogenic Headache was better than C2 paravertebral nerve block.

颈源性头痛，是由颈椎或颈部软组织的器质性或功能性病损所引起的以慢性、单侧头部疼痛为主要表现的综合征［1］。在颈椎第2横突穿刺注射消炎镇痛药物对大多数颈源性头痛患者具有良好的疗效［2-3］。为探讨水针刀联合颈2椎旁注射治疗颈源性头痛的临床疗效，该院方便选取2013年6月—2014年4月收治的58例患者为研究对象，现报道如下。
