高血压患者心率变异性及心率减速力变化

目的 探讨高血压患者心率变异性（heart rate variability，HRV）及心率减速力（deceleration capacity of heart rate，DC）变化。方法 选取80例健康体检和64例高血压患者进行24 h动态心电图监测，分析比较2组的HRV时域指标:全部窦性心搏R-R间期标准差（standard deviation of normal-to-normal intervals，SDNN）、R-R间期平均值的标准差（standard deviation of the average normal-to-normal intervals，SDANN）、相邻R-R间期差值的均方根（root mean square of successive differece，RMSSD）、相邻R-R间期差值＞50 ms的百分比（proportion of number of pairs of adjacent normal-to-normal intervals by more than 50 ms，PNN50）、三角指数和DC，探讨高血压患者HRV与DC的相关性，以及较健康体检者差异是否有统计学意义。结果 高血压患者的SDNN、SDANN、三角指数、DC均较健康体检者减低，差异有统计学意义（P＜0.01），RMSSD、PNN50差异无统计学意义。高血压组DC与SDNN、SDANN、RMSSD、PNN50均呈正相关，差异有统计学意义（P＜0.01）。结论 高血压患者HRV和DC减低，自主神经功能受损；高血压组DC与HRV时域指标呈正相关。

［Abstract］Objective To investigate changes of heart rate variability（HRV）and deceleration capacity of heart rate（DC）in patients with hypertention.Methods Eighty cases of healthy people and 64 cases of patients with hypertention underwent 24 hours ambulatory electrocardiographic recording to analyze the correlationns of DC and HRV time domain index（SDNN，SDANN，RMSSD，PNN50），HRV triangular index（TI）in hypertention group，and compared these items between the two groups.Results The SDNN，SDANN，TI，DC in patients with hypertention significantly decreased than those in healthy people（P＜0.01）.However，there was no significant difference of RMSSD and PNN50 between the two groups.The DC was positively related with SDNN，SDANN，RMSSD and PNN50 in hypertention group（P＜0.01）.Conclusion HRV and DC in patients with hypertention decreased，there was damage with autonomic nerve function in those patients.DC was positive associatied with HRV time domain index in hypertention group.

高血压是临床的常见病和多发病，其心、脑、肾等靶器官损害严重影响着患者的生活质量。自主神经系统在高血压的发生、发展中起着重要作用［1］。心率变异性（heart rate variability，HRV）和心率减速力（deceleration capacity of heart rate，DC）是反映自主神经功能的2个指标，HRV减低或DC异常说明自主神经功能受损。本研究通过对患者的24 h动态心电图监测结果进行离线分析，获得HRV和DC的各项参数，并进行比较和相关性分析。

1 资料与方法

1.1一般资料 选取2014年1月—2015年7月海军总医院收治的64例原发性高血压患者为研究对象（试验组），其中男性 29例、女性 35例，年龄（60.41±15.44）岁，所有患者均符合《中国高血压防治指南2010》中高血压相关诊断标准。选取同期80例健康体检者为对照组，其中男性34例、女性46例，年龄（58.65±16.75）岁，随测血压＜130/80 mmHg。……
