老年抑郁症患者社区综合护理干预的效果分析

目的探讨对老年抑郁症患者进行社区综合护理干预的效果评价。方法将2013年6月30日～12月31日在浙江省立同德医院覆盖的社区内的门诊随访的老年抑郁症患者96例随机分为干预组和对照组，每组48例，两组均行常规康复治疗、定期家访或电话随访治疗，干预组在此基础上给予综合护理干预，内容包括健康教育、健康生活方式指导等，并在治疗前及第2、4、8周末进行抑郁自评量表和社会功能缺陷量表评定。结果 在第4周和第8周结束时，综合护理干预组的抑郁自评量表评分及SDSS评分显著低于对照组（P＜0.05）。调查中发现干预组的依从性显著好于对照组（P＜0.05）。结论 在系统药物治疗的基础上对社区内的老年抑郁症患者进行综合护理干预，可以有效缓解患者的抑郁症状，并可以显著改善患者的社会功能。

［Abstract］Objective To explore the effectiveness of comprehensive nursing intervention on elderly patients with depression in community.Methods A total of 96 elderly patients with depression，being outpatient follow-ups of the covered communities in our hospital ranged from Jun.30，2013 to Dec.31，2013，were randomly divided into the study group and the control group，and 48 in each.Both groups were treated with traditional rehabilitation therapy，online interview or home interview，meanwhile patients in the study group received comprehensive nursing intervention.The scores of SDS and SDSS were assessed at the start point of the first week，and the end of points of the second week，fourth week and eighth week.Results Scores of SAS and SDSS at the end point of the fourth and eighth week were much lower in the comprehensive nursing intervention group（P＜0.05），the compliance of patients in the study group were significantly higher comparing to the control group（P＜0.05）.Conclusion Depression symptoms and social function of patients could be improved on the basis of systematic medicine treatment plus comprehensive nursing intervention in community.

随着社会发展和经济进步，抑郁症的发病率逐年增加，其造成的经济负担也越来越严重，根据预测，到2020年将成为全球范围内第二大导致残疾的疾病［1］。由于抑郁症极易复发，可严重损害患者的工作能力和生活能力，尤其是老年人。因此，帮助老年抑郁症患者减少疾病症状带来的影响，降低其复发率具有重要意义［2］。我们对社区内老年抑郁症病人正常药物治疗和常规康复的基础上，进行了综合护理干预并进行了效果评价。……
