医保与非医保患者住院费用比较及其影响因素分析

目的 比较医保与非医保患者住院费用并对其影响因素进行分析。方法 选取该院2014年1月—2016年2月收治的500例骨科医保患者及500例非医保骨科患者作为研究对象，对其住院费用进行比较并采用Logistic回归分析法对其影响因素进行分析。结果 医保患者各项住院费用高于非医保患者，差异有统计学意义（P＜0.05）；Logistic回归分析结果提示，病种、是否手术、院内感染为影响医保患者住院费用的独立影响因素（OR：4.332、4.451、4.689，OR95%CI：1.456～5.332、1.549～5.491、1.620～5.642，P＜0.05）。结论医保制度的实施并未遏制医疗费用的不合理增长，并在一定程度上刺激了医疗消费，而病种、是否手术、院内感染为影响医保患者住院费用的独立影响因素。

［Abstract］Objective To compare the health care and hospital patients without health care costs and to analyze its influence factors.Methods From January 2014 to February 2014 were the 500 medicare patients，500 cases of health-care orthopaedic patients as the research object，carries on the comparison to the hospitalization cost and Logistic regression analysis to analyze its influence factors.Results Health is higher than the patients without health insurance，the hospitalization expenses of patients with statistically significant difference（P＜0.05）；Logistic regression analysis results suggest，whether disease，surgery，hospital infection for independent impact factors affecting hospitalization expenses（OR：4.332，4.451，4.689，OR95%CI：1.456～5.332，1.549～5.491 and 5.491～5.642，P＜0.05）.Conclusion The implementation of the health care system does not contain the unreasonable growth of medical expense，and to a certain extent，to stimulate the medical consumption，and disease，whether surgery，hospital infection for independent impact factors affecting hospitalization expense.

医疗保险制度（Medical Insurance System）是我国为了解决百姓看病问题，基于保险原则筹集、分配和使用的一种医疗保险基金制度，为社会保险的重要构成部分并成为当前世界范围内应用最为普遍的一种卫生费用管理模式［1］。然而，近些年来随着参保人群的不断扩大，医疗费用增长过快问题已经引起了社会各界的普遍关注，并且成为当前亟待解决的现实问题。如何遏制过快增长的医疗费用，促使医疗资源得到更加合理的使用成为医学界研究的全新课题。鉴于此，该次研究围绕医保与非医保患者住院费用比较及其影响因素展开深入分析，现将结果报道如下。……
