个体化护理在肿瘤PICC置管患者中的应用

目的研究个体化护理对接受PICC置管术的肿瘤患者的影响。方法在三甲医院一年的进修和学习期间，选取120例接受PICC术的肿瘤患者，随机分成对照组和干预组（n=60）。对照组患者按照常规方式进行护理，干预组采用个体化的护理方式。观察置管期间有无异位、堵管、药液渗漏和黏膜损伤、静脉炎等的发生。结果经过临床观察，采用个体化护理的干预组发生异位（0例）、堵管（1例）、药液渗漏（2例）、黏膜损伤（4例）、静脉炎（3例）、静脉血栓（1例）等相关并发症的数量均明显小于对照组（P＜0.05）。插管成功率57（95.0%）明显高于对照组（P＜0.05）。结论在临床上，对行PICC置管术的肿瘤患者采用个体化护理方法有助于减少并发症的发生，增加插管成功率，减轻患者痛苦，提高治疗效率，值得推广。

［Abstract］Objective To research the effectof individualized nursing on tumor patientswith PICC catheterization.M ethods 120 cases of tumor patients with PICC operation during the further education and learning of one year in the AAA hospital were randomly divided into two groupswith 60 cases in each，the control group adopted conventionalmethod of nursing，the international group adopted individualized nursing method，and whether the ectopia，tube plugging，drug leakage，mucous injury and phlebophlogosis during the catheterization period occurred or not were observed.Results After clinical observation，the ectopia，tube plugging，drug leakage，mucous injury，phlebophlogosis and venous thrombus respectively occurred to 0，1 case，2 cases，4 cases，3 cases and 1 case in the interventional group，the number of case with relevant complications was obviously fewer than those in the control group，P＜0.05，the success rate of cannula was obviously higher than that in the control group，P＜0.05.Conclusion In clinic，the individualized nursingmethod for tumor patientswith PICC catheterization contributes to reducing the occurrence of complications，increasing the success rate of cannula，relieving pains of patients and improving the treatment efficiency，which isworth promotion.

肿瘤患者在临床治疗中大都需要药物化疗，并且化疗药物一般毒副作用较强，往往对药物进入体内的静脉处产生较强的刺激作用，导致伤口处黏膜损伤和静脉炎［1］。经外周静脉穿刺中心静脉置管（PICC）减少化疗药物对外周静脉血管的刺激，提高了化疗的效率［2］。为了提高患者的插管成功率，提高肿瘤患者的护理质量，笔者进行修所在某三甲医院对2012年全年接受PICC术的120例肿瘤患者进行了护理方法的研究，现报道如下。……
