腹腔镜辅助胃癌根治手术治疗进展期胃癌的短期疗效评价

目的观察分析腹腔镜辅助小切口胃癌根治术对进展期胃癌的短期疗效评价。方法选取2013年2月—2014年12月鹤壁市人民医院普外科病区收治的进展期胃癌患者61例，其中腹腔镜手术治疗31例，开腹手术30例，两组患者均行D2根治术。治疗过程中及治疗结束后，对两组患者的基本资料和临床结局行统计学处理。结果:行腹腔镜手术治疗组，可检获的淋巴结数量（37.99±9.89）枚，明显多于开腹组（19.89±15.36）枚，手术时的切口长度（7.3±1.6）cm也明显短于开腹组（15.4±3.1）cm；患者的住院总天数（9.8±1.7）d也明显短于开腹组（11.3±2.0）d，数据均行t检验，显示差异有统计学意义（P＜0.05）。结论腹腔镜辅助胃癌根治手术即可较好地清除淋巴结，缩短患者住院时间，也不会增加患者的经济压力。

［Abstract］Objective To observe and analyze the laparoscopic assisted small incision gastric cancer radical operation for advanced gastric cancer short-term evaluation.Methods A total of February 2013 to 2014 December Hebi City People's hospital general surgery wards were progression in patients with gastric cancer in 61 cases，including laparoscopic surgery in the treatment of 31 cases，laparotomy in 30 cases，two groups of patients underwent D2 radical operation.After the course of treatment and after treatment，basic data of two groups of patients and clinical outcomes for statistical analysis.Results Laparoscopic surgery in the treatment group，seized the lymph nodes（37.99±9.89）gold，significantly more than the laparotomy group（19.89±15.36）medals，surgical incision length（7.3±1.6）cm was significantly shorter in the laparotomy group（15.4±3.1）cm in；patients hospitalized days（9.8±1.7）days was significantly shorter in the laparotomy group（11.3±2.0）days.Data were performed t test shows there is a statistically significant difference（P＜0.05）.Conclusion Laparoscopic assisted gastric cancer radical surgery can be better removed lymph nodes in the patients，shorten hospitalization time，and will not increase the economic pressure of the patients.

在我国胃癌是最常见的消化系统肿瘤之一，且由于国情不同，我国的胃癌患者又以进展期患者居多［1］。日本医生Kitano于1994年首先报导了腹腔镜辅助胃切除的方法治疗胃癌［2］。腹腔镜技术应用于临床20余年来已取得长足发展，但是腹腔镜辅助小切口手术治疗进展期胃癌仍有较大争议。质疑者主要担忧其淋巴结清扫范围及程度是否充足，且该方法手术时间长，费用较高等。选取2013年2月—2014年12月鹤壁市人民医院普外科病区收治的进展期胃癌患者61例，进行研究观察分析以期了解腹腔镜治疗进展期胃癌的短期疗效和临床应用价值，现报道如下。……
