探讨新型及特殊保持装置改良式热压膜联冠透明器在口腔正畸中的应用效果

目的探讨口腔正畸治疗中新型及特殊保持装置的临床应用效果。方法入选该院2013年8月—2014年8月完成口腔正畸治疗的患者共60例，随机分为对照组和观察组各30例，对照组治疗后佩戴Hawley保持器，观察组佩戴改良式热压膜联冠透明器，比较两种保持装置的应用效果。结果两组PAR指数在牙齿排列、覆牙合、颊舌向牙合关系及垂直向牙合关系方面的差异比较具有统计学意义（P＜0.05）。结论与Hawley保持器相比，正畸治疗后佩戴改良式热压膜联冠透明器的保持效果更加明显，值得临床进一步使用和推广。

［Abstract］Objective To study the clinical application effect of new and special retaining device in orthodontics.Methods 60 cases of patients finishing orthodontics treatment in our hospital from August 2013 to August 2014 were selected and randomly divided into two groups with 30 cases in each，the control group wore Hawley's retainer after treatment，the observation group wore modified transparent full-arch wraparound retainer，and the application effects of the two retaining devices were compared.Results The differences in the tooth arrangement，overbite，buccal and lingual teeth relationship and vertical occlusal relationship between the PAR indexes of the two groups had statistical significance by comparison，P＜0.05. Conclusion The retain effect of wearing modified transparent full-arch wraparound retainer is more obvious after the orthodontics treatment compared with that of Hawley's retainer，which is worth further application and promotion in clinic.

口腔牙颌畸形是牙颌发育不良的主要表现，其发病机制十分复杂，主要受先天或后天因素的影响比较明显。儿童是牙颌畸形的好发群体，虽然牙颌畸形的发生不易造成严重的功能障碍，但对患者面部外形及美观形象的影响十分严重［1］。随着社会的发展及医疗水平的提高，人们对美的追求及形象意识不断提高，进一步激发了牙颌畸形患者治疗口腔疾病的欲望。临床一般使用保持器在口腔正畸后进行辅助治疗，而保持器的使用在矫治效果的提高上发挥着重要作用。但由于患者口腔周围组织与软组织、硬组织的改造时间密切相关，治疗时需要选择适宜的保持器进行正畸辅助治疗，才能有效提高口腔正畸的维持效果［2］。……
