集束化策略对重症监护室实习医生手卫生依从性干预效果分析

【摘要】目的 分析集束化策略对重症监护室实习医生手卫生依从性的干预效果，强化实习医生手卫生意识，提高其手卫生的依从性。方法将本科室2015年8月～2016年1月来自不同院校的60名实习医生作为研究对象，采用集束化策略对其手卫生依从性进行干预，干预前后对其手卫生依从率，手部细菌菌落数进行统计分析。结果 干预前后重症监护室实习医生手卫生依从率及手部细菌菌落数比较，差异有统计学意义（P＜0．05）。结论 集束化策略能提高重症监护室实习医生的手卫生依从性。

Objective To analyze intervention effect of the cluster strategy in ICU practice doctor hand hygiene compliance，strengthening practice physician awareness of hand hygiene，from semi professional to improve the hand hygiene compliance． Methods The undergraduate laboratory from August 2015 to January 2016 60 students from different colleges and universities as the research object． Using the cluster strategy intervention on the compliance of hand hygiene，before and after the intervention，the hand hygiene compliance rate，the number of hand bacterial colonies for statistical analysis．Results Before and afterintervention，difference of ICU practice doctor hand hygiene compliance rate and hand bacteria colony number was statistically significant（P＜0．05）． Conclusion The cluster strategy can improve the hand hygiene compliance of the intensive care unit．

手卫生依从性是指医护人员能够坚持必要的手卫生次数、步骤、时间范围都符合规定［1］。重症监护室主要收治全院的危重症患者，多数基础病情严重，免疫力低，接受侵入性操作多，住院时间长，是医院感染的高危人群，感染发生率高于同期全院发生率医院感染不仅危害患者的生命，延长患者住院周期，而且也给医院带来损失。医护人员的手是医院感染中最重要的传播媒介，医院感染病例中多数为接触性传染。有研究表明医务人员手卫生是预防医院感染最基本、最直接、最经济和最有效的措施［2］。本科室每年承担大批实习医生的教学任务，实习医生刚进入临床实习，更多地参与基础性诊疗工作，而其手直接触病区环境及物体表面，增加了医院感染的机会。……
