Er：YAG激光用于牙颈部楔状缺损的临床观察

【摘要】目的 探究应用Er：YAG激光治疗楔状缺损伴牙本质过敏的效果。方法 选取2014年1月～2015年12月于一汽总医院口腔科门诊就诊的109例患者，随机分成两组，即实验组用Er：YAG激光制洞加激光脱敏治疗法（60颗），对照组用机械预备加脱敏剂的方法（49颗）。比较两组患牙的治疗效果，牙本质过敏症状的改善结果。结果 两组治疗后修复体的表面情况、边缘着色、是否有继发龋和牙龈炎症对比检验，P＞0．05，差异没有统计学意义；两组治疗后的牙本质敏感度，P＜0．05，差异有统计学意义。结论 结合Er：YAG激光的治疗特点，证实激光在光固化复合树脂治疗有牙本质过敏的楔状缺损中有较好的临床应用价值。

Objective To observe the effect of Er：YAG Laser in the treatment of teeth with wedge-shaped defect and dentin sensitivity． Methods 109 patients who visited the FAW hospital between January 2014 and December 2015 had been chosen，and randomly divided into two groups． The experimental group were treated by the Er：YAG laser（60 cases），the control group were treated with the traditional method and desensitizer （49 cases）． The effect in desensitizing of the two groups were compared． Results The results of the surface of the restoration situation，edge coloring，secondary caries and gum inflammation were tested，P＞0．05，without statistical significance． The result of dentin sensitivity of the two groups were with statistical significance（P＜ 0．05）． Conclusion Er：YAG laser treatment is useful in the treatment of teeth with wedge-shaped defect and dentin sensitivity．

楔状缺损是指牙齿唇、颊面牙颈部的牙体硬组织缓慢磨耗而致的缺损，呈“V”状，其好发生于前磨牙，尤其是第一前磨牙，并且在人群中的发病率较高，尤其是在中老年人中十分常见，患病率高达47.5%～60.0%，最常见的症状是牙本质过敏。临床多采用光固化复合树脂充填治疗，但因其缺损的部位及形态特殊，充填治疗时易引起牙髓过敏反应及不易固位，为探讨更有效的治疗方法，本研究拟对Er：YAG激光在临床备洞、脱敏，观察患者的疼痛反应和术后过敏症状，以及复合树脂充填的成功率，以期获得更好的治疗楔状缺损的方法。
