不同并发症数量的老年2型糖尿病住院病人生活质量及影响因素分析

摘要:[目的]了解不同并发症数量的老年2型糖尿病住院病人生存质量的影响因素。[方法]将西安市3所医院190例老年2型糖尿病病人按照并发症数量不同分成两组，采用一般情况调查表、糖尿病特异性生活质量量表(DSQL)进行调查。[结果]病人生活质量总分为(58.560±11.805)分，生理因素(25.970±7.078)分，心理精神因素(20.000±4.654)分，社会关系(6.840±2.093)分，治疗(5.750±1.674)分；有4种及以上并发症的病人生活质量总分及各维度得分均高于有3种及以下并发症的病人，其中生活质量总分、生理因素、社会关系得分比较差异有统计学意义(P<0.05)；多元线性逐步回归分析表明，并发症数量在4种及以上的病人总体生活质量影响因素有收入水平、照顾需求、病程和体重指数；并发症在3种及以下的病人总体生活质量的影响因素有睡眠质量、收入水平、年龄和照顾需求。[结论]并发症数量较多的病人生活质量较差，要积极预防并发症的发生，建立和谐的人际关系。

Analysis of quality of life and its influencing factors in elderly hospitalized patients with type 2 diabetes mellitus with different number of complications

AbstractObjective:In order to analyze the influencing factors of the quality of life in elderly hospitalized patients with type 2 diabetes mellitus with different number of complications.Methods:A total of 190 cases of elderly patients with type 2 diabetes mellitus in 3 hospitals in Xi’an were divided into two groups according to the number of complications.The general situation questionnaire and the diabetes specific quality of life scale(DSQL) were used to investigate.Results:The total score of the patients’ quality of life was(58.560±11.805).The score of physiological factors was(25.970±7.078).The score of psychological factors was(20.000±4.654).The score of social relations was(6.840±2.093).Treatment dimension score was(5.750±1.674).The total score of quality of life of patients with 4 or more complications and the scores of all dimensions were higher than that in the patients with 3 or less complications.The total score of life quality,physical factor and social relationship were compared,and the difference was statistically significant(P<0.05).Multiple linear stepwise regression analysis showed that the influence factors of overall quality of life of patients with4 or more complications were income levels,care needs,disease duration,and body mass index;influencing factors of overall quality of life of patients with 3 or less complications were sleep quality,income level,age and care needs.Conclusion:The quality of life of patients with a large number of complications was poor.They should actively prevent the occurrence of complications,encourage patients to communicate with others,so as to establish a harmonious interpersonal relationship.

并发症是2型糖尿病病人致残和致死的主要原因，严重影响病人的生活质量。老年糖尿病病人病程长，常伴有多种并发症。楼雪勇[1]报道，我国2型糖尿病病人并发症及其合并症患病率高达72.1%。国内关于糖尿病病人生存质量的研究较多，但关于并发症数量与生存质量间关系的研究较少。本研究对有不同并发症数量的老年2型糖尿病住院病人的生活质量影响因素进行分析，为临床护理工作提供依据。

1对象与方法

1.1研究对象选取2013年3月—8月在西安市3所三级甲等医院内分泌科住院的190例老年2型糖尿病病人。纳入标准：年龄≥60岁；自愿参加研究；符合2010年版《中国2型糖尿病防治指南》2型糖尿病诊断标准；……
