[摘要] 目的 探讨规范化健康管理在社区老年脑卒中患者中的应用效果。 方法 选取2014年1～12月在本社区卫生服务中心就诊且均在本中心建立健康档案的100例老年脑卒中患者作为研究对象，随机分为干预组和对照组，各50例。对照组给予常规管理，干预组在对照组的基础上进行规范化健康管理，比较两组的运动功能评分和生存质量评分。 结果 干预组干预后的上肢与下肢运动功能评分显著高于干预前及对照组干预后，差异有统计学意义（P<0.05）。干预组干预后的生存质量评分显著高于干预前及对照组干预后，差异有统计学意义（P<0.05）。结论 对社区老年脑卒中患者开展规范化健康管理，能够及时准确地掌握患者的信息，规范社区脑卒中管理，提高患者的运动功能及生存质量。

[Abstract] Objective To explore the application effect of standardized health management in community elderly patients with stroke. Methods 100 elderly stroke patients in our center with established health record from January 2014 to December were selected and randomly divided into the intervention group and the control group，50 cases in each group.The control group was given routine management，the intervention group was given standardized health management on the basis of the control group.The score of exercise function and quality of life in the two groups was compared. Results The score of exercise function of the upper and lower limbs in the intervention group after intervention was higher than that before intervention and in the control group after intervention，with significant difference （P<0.05）.The score of quality of life in the intervention group after intervention was higher than that before intervention and in the control group after intervention，with significant difference （P<0.05）. Conclusion Standardized health management in elder stroke patients in community can timely and accurately grasp the patient information，standardize management of stroke in community，improve exercise function and quality of life of patients.

脑卒中（stroke）是由于急性脑循环障碍导致局限性或弥漫性脑功能缺损的一类疾病，其发病率、致残率、复发率都非常高，严重影响患者的健康及生活质量[1]。脑卒中已经成为威胁我国人民生命安全的第一致死疾病，其经常遗留各种功能障碍甚至躯体残疾，不仅严重影响患者的生存质量，同时也给家庭和社会带来了极大的经济和心理压力。脑卒中患者的康复训练是一个长期持续的过程，随着我国社区卫生服务的发展，脑卒中患者回归社区进行长期规范化管理越来越受到重视，但是目前针对脑卒中患者管理的社区卫生服务体系并不完善[2]。为了完善脑卒中患者的社区健康管理模式，提高脑卒中患者的生存质量，本研究就脑卒中患者规范化健康管理的应用效果进行分析。

1 资料与方法
