摘 要 目的：建立老年糖尿病足溃疡“医院-社区-家庭”延续护理模式并探讨其应用价值。方法：2012年12月—2014年12月对华东医院收治的24例老年糖尿病足溃疡患者进行延续护理，观察患者足溃疡的转归。结果：22例老年糖尿病足溃疡患者创面愈合，占91.7%，愈合时间为36～96d，平均72d。2例患者因病情变化再次入院，溃疡未愈合。结论：“医院-社区-家庭”延续护理模式能有效促进老年糖尿病足溃疡的愈合。

Establishment and application of the extended nursing mode for the elderly diabetic foot ulcers

ABSTRACT Objective： To establish the extended nursing mode of “hospital-community-family” for the elderly diabetic foot ulcers and assess the application value. Methods： Twenty-four cases of the elderly diabetic foot ulcers who were admitted into Huadong Hospital for treatment from December 2012 to December 2014 were given the extended nursing to observe the outcome of the foot ulcers. Results： The foot ulcers of twenty-two patients were healed， accounting for 91.7%， and the healing time ranged from 36 to 96 days， with an average of 72 days. Two patients were re-admitted into the hospital because of the changes of their illness and their ulcers were not healed. Conclusion： The extended nursing mode of “hospital-community-home”can effectively promote the healing of diabetic foot ulcers in the elderly diabetic patients.

随着我国老龄化程度加剧，60岁以上老年人的比例逐年增加，老年糖尿病患病率也随之增加[1]。糖尿病足溃疡是老年糖尿病常见和严重的并发症之一。据报道随着病程的发展，糖尿病患者最终有30%并发足病，而在65岁以上老年患者中的发病率更高[2]。老年糖尿病患者由于增龄、高血糖、下肢供血差等因素，足溃疡的愈合不同于一般伤口，需要较长时间的专业治疗和护理。由于足溃疡入院诊治患者在病情稳定后即可出院，此时患者溃疡面仍需接受专业、持续的创面护理，因此迫切需要社区、家庭的延续性护理服务。本文采用“医院-社区-家庭”延续护理模式对糖尿病足溃疡患者进行护理，取得良好效果，现报道如下。

1 资料与方法

1.1 一般资料

选择2012年12月—2014年12月复旦大学附属华东医院内分泌就诊的老年糖尿病足溃疡患者24例，其中男性6例，女性18例，年龄62～93岁，平均（79.12±8.62）岁。24例中，Wagner分级1级5例，2级13例，3级6例；……
