家庭医生制度下老年人压疮的规范护理探讨

摘要目的：探讨在家庭医生制服务下长期卧床老年压疮患者家庭内规范护理后的效果。方法：收集2013年社区卫生服务中心长期卧床或生活不能自理的不同程度压疮老年患者49例，处于淤血红润期11例，炎症浸润期18例，溃疡期Ⅰ度14例，溃疡期Ⅱ度6例。对患者进行1年的家庭内压疮规范护理。结果：49例患者中压疮治愈41例，占83.6%，好转7例，占14.2%，无效1例，占2%。结论：由患者、家属、家庭医生社区护士共同参与的医疗保健服务模式，有利于长期卧床老年压疮患者的病情康复。

Exploration of the standardized nursing of the elderly pressure ulcers under the family doctors’ system

ABSTRACTObjective: To study the effect after the standardized nursing of the long-term bedridden elderly patients with pressure ulcers at home under the service of the family doctors’ system. Methods: Forty-nine cases of the elderly patients with pressure ulcers who were in the long-term bedridden or had no ability to manage their life by themselves in the different degree in the community health service center in 2013 and among them, 11 cases were in the congestion ruddy stage, 18 ones in the inflammatory infiltration stage, 14 cases in the ulcer stage I degree and 6 ones in the ulcer stage II degree. All the patients were cared with the standardized nursing of pressure ulcers at home for one year. Results: Among 49 cases with pressure ulcers, 41 ones were cured, accounting for 83.6%, 7 cases improved, 14.2% and one case had no effect, 2%. Conclusion: The medical care service model participated with the patients, family members, family doctors and community nurses is of benefit to the disease condition rehabilitation in the long-term bedridden elderly patients with pressure ulcers.

随着人口老龄化的加快，社区中长期卧床生活不能自理的老年患者压疮发生率很高，生活质量下降，并发症及反复住院率较高，多数患者经短期住院治疗后以居家治疗为主。为使这些患者能得到及时、有效、专业的全程治疗和护理，南翔镇社区卫生服务中心在家庭医生制服务基础上，建立家庭病床，由指定医护人员定期查床、治疗、护理。本文总结2013年南翔镇社区卫生服务中心对家庭内压疮老年患者规范护理的效果。
