针灸治疗压力性尿失禁60例临床观察

【摘要】目的：观察中医针灸治疗女性盆底功能障碍对于性生活质量的影响。方法：选取女性压力性尿失禁患者120例为观察对象，根据随机抽签原则分为治疗组与对照组各60例，对照组给予常规物理运动疗法进行干预，在此基础上治疗组加用中医针灸干预，比较两组的临床疗效和症状改善情况。结果：治疗后，治疗组与对照组的总有效率分别为96.7%和76.7%，治疗组的临床疗效明显优于对照组(P<0.05)；两组治疗后的压力性尿失禁受控评分均明显低于治疗前，其中治疗组评分明显低于对照组(P<0.05)；治疗组的性欲、性高潮、性心理、性交愉悦评分都明显高于对照组(P<0.05)；治疗组的逃避、限制、心理社会影响、自我困扰评分明显低于对照组(P<0.05)。结论：中医针灸治疗女性盆底功能障碍能有效改善临床症状，提高治疗疗效，能促进性生活质量与生活质量的提高，值得推广应用。

Abstract:ObjectiveTo discuss the quality of life effects acupuncture treatment for the female pelvic floor dysfunction. Methods 120 female urinary Stress urinary incontinence based on a random draw were equally divided into treatment group and control group, the control group were received conventional physical exercise therapy intervention,and the treatment group were add the acupuncture intervention,the treatment periods were four weeks.Results After evaluation,the efficiency of the treatment and control groups were 96.7% and 76.7% respectively, the efficiency of the treatment group were significantly higher (P<0.05).After treatment,the urinary Stress urinary incontinence controlled scores were significantly lower than before treatment,and the urinary Stress urinary incontinence controlled scores in the treatment group were significantly lower than the control group (P<0.05).After an investigation after treatment,the libido,orgasm,sexual psychology,sexual pleasure scores in the treatment group were significantly higher (P<0.05).The evade,limitations,psychological and social impact of self-obsession scores in the treatment group were significantly lower than the control group (P<0.05).ConclusionAcupuncture treatment for female pelvic floor dysfunction can improve clinical symptoms,improve treatment efficacy,it can promote the improvement of quality of sex life and quality of life that should be widely applied.

女性盆底是由封闭骨盆出口的多层肌肉、筋膜、尿道、阴道、盆底肌肉群、韧带及神经构成复杂的系统，它们之间的相互作用可承托并保持子宫、膀胱、尿道和直肠等盆腔脏器处于正常位置。压力性尿失禁(Urinary Incontinenc，UI)就是由于女性盆底功能障而导致的一种常见疾病，可发生于所有年龄组，其患病率随年龄增长而升高[1-2]。压力性尿失禁严重危害了女性患者的身心健康，可造成患者行动不便，进而有消沉压抑、孤独、丧失自信等心理障碍，也能促进患者其他疾病的发生。压力性尿失禁的手术治疗对于女性患者有比较大的创伤，在临床上比较少用。当前针灸治疗压力性尿失禁的报道较多，运用不同的手法，均获得较好的效果，但缺乏统一的标准。
