降钙素元与C反应蛋白联合检测在老年人感染性疾病中的应用

[摘要]①目的分析降钙素原(PCT)与C反应蛋白(CRP)联合检测对老年人感染性疾病的应用。②方法比较分析重度细菌性感染(39例)、一般细菌性感染(52例)、病毒性感染(48例)和非感性疾病(60例)PCT、CRP检测结果的差异。③结果重度细菌性感染(I组)、一般细菌性感染(II组)PCT水平均比病毒性感染组(III组)及非感染性疾病对照组(IV组)明显增高，差异有统计学意义(P<0.05)；CRP在I、II、III组上的水平均比IV组高，差异有统计学意义(P<0.05)，但这3组两两之间的CRP存在的差异没有统计学意义(P>0.05)。④结论PCT与CRP均与老年人感染性疾病密切相关，联合检测有更大的临床应用价值。

ABSTRACT[]ObjectiveTo analyze application on detection of detection of Procalcitoin(PCT)combined C-reactive protein(CRP)in infectious disease of elderly people.MethodsTo study differences between testing results of PCT and CRP with severe bacterial infections(39 cases),general bacterial infections(52 cases),viral infection(48 cases)and non-inductive disease(60 cases).ResultsPCT levels of severe bacterial infections(group I)and general bacterial infections(group II)significantly higher than those of viral infection group(group III)and control group(group IV),difference was statistically significant(P<0.05);CRP levels in group I,II,III were higher than that in group IV,the difference was statistically significant(P<0.05),but the differences among the first three groups of CRP have no statistical significance(P>0.05).ConclusionPCT and CRP are closely associated with the elderly infectious diseases,the combined detection has greater value.

感染是老年患者最常见的疾病并发症以及导致死亡的原因之一，而且感染的临床症状复杂却不典型，患者的自觉症状轻、有疲劳感、发热不明显或低热，病初不易觉察又多样化。目前，临床上最通用的方法是根据外周血象和细菌定性培养判断患者是否合并了感染，但是单纯关注外周血象的变化来判别临床诊断和治疗还是不足，而细菌定性培养又有一定的时间局限性，不利于及时诊断。血清C反应蛋白(CRP)是一种极灵敏的急性时相反应蛋白，是反映全身性炎症反应的非特异性标志物。临床上已将CRP作为一个感染性疾病的诊断和疗效观察的重要指标并应用于急诊。降钙素原(PCT)历经近20年的研究和实践证明与感染和脓毒血症的相关性很好，有助于临床鉴别感染性疾病与非感染性疾病。……
