复方中药联合卡培他滨同步维持治疗晚期结直肠癌

摘要：目的评价复方中药联合卡培他滨在晚期结直肠癌维持期的疗效。方法将符合晚期结直肠癌维持治疗诊断标准的100例患者随机分为治疗组和对照组,各50例。治疗组：卡培他滨按照每日总剂量(常规用量每2 000 mg/m2)分早、晚各1次，饭后30 min口服，同步复方中药每日1剂，分2次，早、晚饭后1 h服用。连续用药2周停药1周。对照组:单服卡培他滨。2组均持续用药至肿瘤进展或出现无法耐受的不良反应为止。结果治疗组和对照组的中位无疾病进展时间(PFS)分别为6.4个月和2.5个月，2组比较有显著性差异(P<0.05);与对照组比较,治疗组可以明显提高晚期结直肠癌患者的NK细胞及CD4/CD8(P<0.05)，可以明显改善患者的卡氏评分(P<0.01)。结论复方中药联合卡培他滨同步维持治疗晚期结直肠癌具有延长患者PFS、改善免疫功能、提高生活质量的作用。

Maintenance treatment of compound Chinese medicine combine with

capecitabine on advanced colorectal cancer

Abstract:ObjectiveTo evaluate the efficacy of compound Chinese medicine and capecitabine for the maintenance therapy in patients with advanced colorectal cancer.MethodsThe 100 patients who with the diagnostic criteria for maintenance treatment of advanced colorectal cancer,were randomly divided into a treatment group and a control group,50 patients in each group.The treatment group was taken oral administration (normal dosage 2 000 mg/m2 daily) 0.5 h after meals one in the morning and one in the evening and synchronous compound Chinese medicine daily 1 agent,points 2 times,1 hour after meal,after dinner for continuous two weeks then for one-week interval.The control group was only given capecitabine.The two groups continued to take medicine till cancer progression or side effects beyond sufferance.ResultsThe median progression free survival (PFS) in the treatment group was 6.4 months,while in the control group was 2.5 months,which had significant difference (P<0.05);Compare with the control group,the treatment group could apparently improve NK cells and CD4/CD8 (P<0.05),as well as improve the patients’ Karnofsky performance score (P<0.01).ConclusionThe maintenance therapy by compound Chinese medicine and capecitabine in patients with advanced colorectal cancer can effectively prolong the PFS,improve immune function and enhance the quality of life.

我国结直肠癌发病率逐年上升，晚期肿瘤转移导致超过一半的结直肠癌患者死亡。目前，对于转移性结直肠癌治疗多以化疗为主，但是化疗最佳的模式和持续时间仍无统一意见[1]。为了更加有效的延长结直肠癌患者的生存期、提高其生活质量，对晚期结直肠癌强烈化疗得到控制后,采用有效、低毒的化疗药物维持治疗是近年来的研究热点[2]，维持治疗对于晚期结直肠癌仍无统一标准的治疗方案，国内报道较少。……
