[摘要] 目的 探究外固定架和使用内固定术在治疗创伤骨科中四肢骨折患者的应用和效果，总结临床经验。 方法 选取2010年3月—2014年7月该院进行创伤骨科治疗的患者180例创伤骨科四肢骨折的患者为对象，90例患者采用使用外固定架固定治疗，90例采用内固定治疗。回顾性分析两种治疗方法的手术时间、术中出血量、并发症等情况，总结临床效果及经验体会。结果 观察组手术时间（22.42±4.64）min、术中出血量（278.31±28.61）mL、切口长度（1.52±0.63）cm、愈合时间（21.52±4.72）及并发症发生率（22.22%）显著低于对照组（P<0.05）；观察组总优良率（90.00%）高于对照组（65.56%），差异有统计学意义（P<0.05）。 结论 外固定架治疗创伤骨科四肢骨折的患者时，具有突出优点，可以有效帮助患者创伤部位恢复到健康状态，可以在临床上实施推广。

Observation of the curative effect of external fixation in the Department of orthopedics and trauma fractures in extremities

[Abstract] Objective To explore the use of external fixation and internal fixation in the treatment of traumatic limbs fracture in the application and effect of Department of orthopedics patients， summarize the clinical experience. Methods from 2010 March to 2014 July in our hospital for treatment of 180 patients of trauma department of orthopedics department of orthopedics and trauma patients as the object of limb fractures， 90 cases were treated by the use of external fixation in the treatment of 90 cases， treated with internal fixation. A retrospective analysis of two kinds of therapeutic methods of operation time， amount of bleeding during the operation， complications， summarize the experience of clinical effect and experience. Results in the observation group， operation time （22.42±4.64）， the amount of bleeding during operation （278.31±28.61）， incision length （1.52±0.63）， healing time （21.52± 4.72） and the incidence of complications （22.22%） was significantly lower than the control group （P<0.05）； the total excellent and good rate of the observation group （90%） is higher than the control group （65.56%）， the difference was significant （P < 0.05）. Conclusion Fracture fixation in the treatment of limb trauma department of orthopedics conclusion patients， has obvious advantages， can effectively help the patients recover trauma to health， can be implemented in clinical practice.

创伤骨科中，由于外固定架操作简单，治疗过程中创伤小，骨折愈合时间短，术后并发症少而比较广泛的应用于临床[1-2]。对未发生致命性骨骼创伤的病患，外固定架可快速、简便、高效地固定创伤部位，为重症患者择期手术治疗赢得时间[3]。内固定使用金属螺钉、骨板等物直接在断骨内将断骨连接[4]，容易引发感染及二次伤害。所以如何选择有效的治疗，关系到患者的生命安危及术后生活质量。该研究就2010年3月—2014年7月在该院进行创伤骨科治疗的患者180例创伤骨科四肢骨折的病患进行分析，比较外固定架和内固定架的效果，现报道如下。endprint

1 资料与方法

1.1 一般资料

选择该院创伤骨科收治的180例四肢骨折患者，其中男97例，女83例，年龄25～56 岁，平均（34.12±12.86）岁。致伤原因：工作机械伤30例，高处跌落伤48例，交通伤89例，其他意外损伤13例。骨折类型：上肢骨折78 例，其中前臂骨折43例，肱骨骨折35例；……
