[摘要] 目的 分析并研究老年人股骨粗隆部粉碎性骨折采用人工髋骨关节置换治疗的临床效果。方法 将2012年6月—2014年4月收治的40例患有股骨粗隆部粉碎性骨折患者作为临床研究对象，采用随机数字法将其分为两组，对照组采用常规方式髋螺钉固定治疗，实验组采用人工髋关节进行治疗，对两组的治疗效果和安全性进行比较。结果 实验组采用人工髋关节置换治疗的总有效率为90.0%，对照组采用常规方式治疗的的总有效为65.0%，其中实验组明显高于对照组的总有效率25.09%，差异有统计学意义（P<0.05）。 结论 对于患有股骨粗隆粉碎性骨折的患者采用人工髋关节置换治疗的效果较好。患者术后生活工作能力显著增强，患者和家属的满意度高，值得临床进一步推广使用。

[Abstract] Objective To analyze and study the clinical effect of artificial hip joint replacement on treating intertrochanteric comminuted fractures of elderly patients. Methods Selected 40 patients with intertrochanteric comminuted fracture patients as clinical research object， they were randomly divided into two groups， the control group used the conventional way of hip screw fixation ， experimental group used artificial hip joint， compared two groups curative effect and safety. Results The total efficiency of experimental group used hip arthroplasty was 90%， and 65% in the control group used the conventional way ， the total efficiency of experimental group was significantly 25.09% higher than control group， the difference had statistically significant （P<0.05）. Conclusion Artificial hip joint replacement has better efficiency for patients with intertrochanteric comminuted fracture. It significantly enhances patients ability to live and work after surgery，satisfaction of patients and family is high，so it is worth to be popularized in clinic.

股骨粗隆粉碎性骨折是老年骨折中一种常见的骨折，一般主要是患者由间接外力导致，下肢突然性发生一定的扭转，而老年人骨质疏松，当患者经过外力下肢突发性的扭转，从而导致骨折[1]。主要临床表现为局部肿胀、疼痛、活动受到一定的限制，由于患者骨折部位发生在粗隆部位，粗隆相对于身体的其他部位血运较为丰富，骨折后不易痊愈，对于患者的正常生活受到了一定的限制，因此，对于患有股骨粗隆部位粉碎性骨折的治疗显得尤为重要[2]。在该研究中，对于2012年6月—2014年4月患有股骨粗隆粉碎性骨折的患者采用两种方法进行治疗，发现实验组组采用人工髋关节置换的治疗方法临床效果较好，现报道如下。

1 资料与方法

1.1 一般资料

采用随机数字表法将该院收治的40位患者分为两组，排除受伤髋关节原有骨折史或先天畸形者合并严重的心血管疾病、脑血管疾病等原发病者。……
