[摘要] 目的 探讨和比较妈富隆和LNG-IUS治疗围绝经期功能失调性子宫出血的临床疗效。 方法 选择2010年12月—2012年12月该院收治的90 例无排卵性功血患者，随机分为妈富隆组 （n= 45）和LNG-IUS 组（n=45） 进行治疗， 对患者接受治疗前及治疗6个月的性激素水平、血红蛋白、月经情况、 体重指数及不良反应等进行统计分析。 结果 妈富隆组和LNG-IUS 组患者经治疗后月经量明显减少， 血红蛋白明显提高 （P < 0.01）； LNG-IUS 组比妈富隆组对减少患者的月经量以及提高其血红蛋白的作用更显著（P < 0.05， P < 0.01）；LNG-IUS组性激素测定中，仅雌二醇水平下降，余性激素水平（FSH、LH、PRL、P）无明显变化 （P > 0.05），妈富隆组FSH、LH、PRL、E2、P水平均降低 （P < 0.01）； 结论 LNG-IUS可抑制雌激素受体在子宫内膜合成，治疗围绝经期功能失调性子宫出血疗效显著，并且对卵巢功能影响较小， LNG-IUS 较妈富隆临床疗效更显著，安全可靠，不良反应小，值得临床推广应用。

[Abstract] Objective To observe and compare the clinical therapeutic effects between marvelon and levonorgestrel intrauterine system（LNG-IUS） on peri-menopausal dysfunctional uterine bleeding. Methods Ninety patients with anovulatory dysfunctional uterine bleeding admitted to the hospitol from Dec 2010 to Dec 2012 were randomly divided into Marvelon group（n=45） and LNG-IUS group（n=45） and treated by Marvelon， LNG-IUS， respectively. The level of sex hormone， hemoglobin， state of menstruation， body mass index（BMI） and adverse reactions of the two groups before treatment and 6 months after treatment were counted and analyzed. Results After treatment， the menstrual blood volume was much less and hemoglobin was much more in both groups（P<0.01）； and compared with the Marvelon group， the menstrual blood volume was less and hemoglobin was more in the LNG-IUS group（P<0.05， P<0.01）； Compared with pretherapy， there were no significant changes in the levels of sex hormones（levels of FSH， LH， PRL and P） in the LNG-IUS group（P>0.05） except the level of estradiol decreased， while in the Marvelon group， the levels of FSH， LH， PRL， E2 and P all decreased（P<0.01）. Conclusion LNG-IUS can inhibit the synthesis of estrogen receptor in the endometrium and has significant effect on peri-menopausal dysfunctional uterine bleeding with little influence on ovarian function. Compared with Marvelon， LNG-IUS has more significant clinical effect， it is more safe and reliable with less adverse reactions， therefore it is worthy of clinical application and promotion.

围绝经期功能失调性子宫出血是临床中常见的妇产科疾病之一。发病的主要原因是调节生殖的神经内分泌机制紊乱，从而导致下丘脑-垂体-卵巢轴调控失常，若不及时治疗容易导致患者雌激素水平降低[1]。围绝经期功血的药物治疗以性激素（其代表药物“妈富隆”）治疗为主，虽近期有一定疗效，但不良反应明显，远期疗效欠佳。近年来该院应用“左炔诺孕酮宫内缓释系统”（LNG-IUS）治疗围绝经期功血，可诱导上皮萎缩，能减少月经血量，缓解月经相关疼痛等取得较好的效果，与手术相比，LNG-IUS花费较小，可以减轻患者的经济负担，因此，该研究以2010年12月—2012年12月该院收治的患者为研究对象，对妈富隆和LNG-IUS治疗功血的疗效、人体血红蛋白、月经量评分、彻底止血时间、控制出血时间以及生殖激素水平等进行观察和比较，现报道如下。……
